前庭功能训练对儿童中枢性协调障碍的康复疗效观察

摘要

Objective To evaluate the effectiveness of vestibular function training combined with traditional exercise therapy in the treatment of children with central coordination disorder.Methods Eighty children with central coordination disorder were randomly divided into observation group and control group according to the random number table.The control group was treated with comprehensive rehabilitation therapy while the observation group was treated with vestibular function training based on the control group.Results The abnormal postural reflex and muscle tension in the observation group were corrected and Controlled,and the development of motor lag was improved.The total effective rate (92.3％) of the observation group was significantly higher than that of the control group (81.6％;P ＜ 0.05).According to the Gesell scale assessment,the DQ mean value of the gross motor,fine motor,adaptive,language and personal social area in the two groups after treatment were all higher than before treatment (P ＜ 0.05).Compared with the control group,there were significant differences in the developmental quotient of the gross motor,fine motor and adaptive area in the observation group (P ＜ 0.05) but there was no statistically significant difference in the language and personal social areas (P ＞ 0.05).Conclusion Vestibular function training combined with conventional rehabilitation therapy has better effect than only use of conventional rehabilitation therapy for children with central coordination disorder.It is an effective method for early rehabilitation of children with cerebral palsy.%目的 评估前庭功能训练与传统的综合运动康复疗法联合治疗中枢性协调障碍患儿的有效性.方法 将80例中枢性协调障碍患儿根据随机数字表法分为观察组与对照组各40例,对照组采用神经发育运动疗法为主的综合康复疗法,观察组在此基础上加用前庭功能训练治疗.结果 观察组患儿异常姿势反射及肌张力得到纠正和控制,运动发育落后得到改善,治疗总有效率(92.3％)明显高于对照组(81.6％),差异有统计学意义(P＜0.05).经Gesell量表评估,两组治疗后大运动、精细运动、适应性、语言、个人-社交能区DQ均值均较治疗前提高(P＜0.05),且两组大运动、精细运动、适应性能区的发育商差异有统计学意义(P＜0.05),而语言、个人-社交能区差异无统计学意义(P＞0.05).结论 前庭功能训练联合常规康复疗法治疗中枢性协调障碍患儿,疗效优于仅使用常规康复疗法,是脑瘫患儿早期康复治疗的一种有效方法.
