弥漫大 B 细胞淋巴瘤患者 MYC/BCL-2共表达情况及其临床意义分析

摘要

目的：分析弥漫性大 B 细胞淋巴瘤（DLBCL）患者 MYC/ BCL-2共表达情况及其临床意义。方法选取2012年1月—2015年1月唐山市人民医院收治的首诊 DLBCL 患者30例，采用免疫组化法检测患者 MYC 和BCL-2表达情况，并分析不同临床特征 DLBCL 患者 MYC/ BCL-2共表达情况。结果 MYC 阳性表达率为70.0%，BCL-2阳性表达率为53.3%，MYC/ BCL-2共阳性表达率为30.0%。不同性别、临床分期、国际预后指数（IPI）评分及乳酸脱氢酶（LDH）水平 DLBCL 患者间 MYC/ BCL-2共阳性表达率比较，差异无统计学意义（P ﹥0.05）；结外受侵、非生发中心型（non-GCB）、未完全缓解 DLBCL 患者 MYC/ BCL-2共阳性表达率分别高于非结外受侵、生发中心型（GCB）、完全缓解患者（P ﹤0.05）。结论 DLBCL 患者 MYC/ BCL-2共阳性表达率较高，且常见于non-GCB、结外受侵、预后不良的DLBCL 患者。%Objective To investigate the coexpression and clinical significance of MYC/ BCL-2 in patients with diffuse large B cell lymphoma. Methods A total of first - diagnosis patients with diffuse large B cell lymphoma were selected in the People's Hospital of Tangshan from January 2012 to January 2015,immunohistochemical method was used to detect the expressions of MYC and BCL-2,and coexpression rate of MYC/ BCL-2 was compared in patients with different clinical features. Results The positive expression rate of MYC was 70. 0% ,of BCL-2 was 53. 3% ,and the positive coexpression rate of MYC/BCL-2 was 30. 0% . No statistically significant different differences of positive coexpression rate of MYC/ BCL-2 was found in patients with different gender,clinical staging,IPI scores or LDH levels(P ﹥ 0. 05);positive coexpression rate of MYC/ BCL-2 of patients with lymphatic metastasis,non - germinal center B cell lymphoma and without complete remission was statistically significantly higher than that of patients without lymphatic metastasis,with germinal center B cell lymphoma and with complete remission,respectively(P ﹤ 0. 05). Conclusion The positive coexpression rate of MYC/ BCL-2 is relatively high in patients with diffuse large B cell lymphoma,and it is commonly expressed in patients with lymphatic metastasis,non - germinal center B cell lymphoma and without complete remission.
