腰大池持续脑脊液引流术在重型颅脑损伤术后脑脊液漏患者中的应用效果分析

摘要

目的 分析腰大池持续脑脊液引流术(CLCFD)在重型颅脑损伤术后脑脊液漏患者中的应用效果.方法 选取2014-2016年东莞市大岭山医院重症医学科收治的重型颅脑损伤术后脑脊液漏患者75例,采用随机数字表法分为常规组39例和CLCFD组36例.常规组患者在开颅术后第2天行腰椎穿刺术以释放脑脊液,CLCFD组患者在开颅术后第2天行CLCFD.比较两组患者脑脊液漏停止时同、治愈率及颅内感染发生率.结果 CLCFD组患者脑脊液漏停止时间短于常规组(P＜0.05).CLCFD组患者治愈率高于常规组(P＜0.05).CLCFD组患者颅内感染发生率低于常规组(P＜0.05).结论 CLCFD在重型颅脑损伤术后脑脊液漏患者中的应用效果良好,可有效缩短脑脊液漏停止时间并降低患者颅内感染发生风险.%Objective To analyze the application effect of continuous lumbar cerebrospinal fluid drainage in postoperative severe craniocerebral injury patients with cerebrospinal fluid leakage.Methods From 2014 to 2016,a total of 75 postoperative severe craniocerebral injury patients with cerebrospinal fluid leakage were selected in the Intensive Care Unit,Dalingshan Hospital of Dongguan,and they were divided into control group (n =39) and observation group (n =36) according to the random number table.Patients of control group received lumbar puncture to release cerebrospinal fluid,while patients of observation group received continuous lumbar cerebrospinal fluid drainage.Stopping time of cerebrospinal fluid leakage,cure rate and incidence of intracranial infection were compared between the two groups.Results Stopping time of cerebrospinal fluid leakage of observation group was statistically significantly shorter than that of control group (P ＜ 0.05).Cure rate of observation group was statistically significantly higher than that of control group (P ＜ 0.05).Incidence of intracranial infection of observation group was statistically significantly lower than that of control group (P ＜ 0.05).Conclusion Continuous lumbar cerebrospinal fluid drainage has certain clinical effect in treating postoperative severe craniocerebral injury patients with cerebrospinal fluid leakage,can effectively shorten the stopping time of cerebrospinal fluid leakage and reduce the risk of intracranial infection.
