电子健康档案对PICC导管门诊患者的管理效果

摘要

目的:分析建立电子健康档案对经外周静脉穿刺置入中心静脉导管(PICC)门诊患者的管理效果.方法:选取2015年4月~2016年4月我院PICC导管门诊行PICC置管患者120例,采用随机数表法等分为观察组和对照组,观察组由PICC健康管理小组建立电子健康档案,基于网络医疗平台以电话、短信、QQ及微信等管理方式进行健康干预,对照组依据传统健康教育模式每月进行1次电话随访.记录两组患者PICC留置时间、留置成功率、非计划性拔管率,应用社会支持量表、导管维护依从性量表、家庭功能评定量表评价其社会支持程度、导管维护依从性、家庭功能,对比两组患者满意度,并记录导管相关并发症发生率.结果:观察组患者PICC留置时间长于对照组,留置成功率高于对照组,非计划性拔管率低于对照组(P<0.05),观察组患者干预后社会支持评分、导管维护依从性评分高于对照组,家庭功能评分低于对照组(P<0.05),观察组患者护理满意度高于对照组(P<0.05),观察组患者干预后导管相关并发症发生率低于对照组(P<0.05).结论:电子健康档案管理有利于提高PICC导管门诊患者的社会支持度、导管维护依从性、家庭功能,延长留置时间,减少非计划性拔管率,同时减少并发症,提高患者满意度,值得在临床推广应用.%Objective:To analyze the management effect of establishing electronic health records on outpatients with peripherally inserted central catheter (PICC)by peripheral venipuncture.Methods:A total of 120 patients who underwent PICC catheterization in the PICC catheterization outpatient clinic of our hospital from April 2015 to April 2016 were selected and divided into observation group and control group by randomly numbers table.The observation group established electronic health records by PICC health management team and they received health intervention by telephone,messages,QQ and WeChat based on the network medical platform.The control group received telephone follow -ups for once a month according to traditional health education model. The PICC retention time, retention success rate and unplanned extubation rate of the two groups were recorded.By application of social support scale,cath-eter maintenance compliance scale and family function rating scale,the degree of social support,catheter maintenance compliance and family function of the two groups were evaluated.The satisfaction of the two groups of patients were compared and the incidence of catheter -related complications was recorded. Results:The retention time of PICC in the observation group was longer than that in the control group.The success rate of retention was higher than that of the control group and the unplanned extubation rate was lower than that of the control group(P<0.05).After the intervention,the social support score and catheter maintenance compliance score of the observation group were higher than that of the control group.The family function score was lower than that of the control group(P<0.05).The patient's satisfaction of the observation group was higher than that of the control group(P<0.05).After the interven-tion the catheter related complications incidence rate of the observation group was lower than that of the control group(P<0.05).Conclusion:The manage-ment of electronic health records is helpful to improve the social support,catheter maintenance compliance and family function of the PICC catheter outpa-tients.It can prolong the retention time and reduce unplanned extubation rate as well as complications and improve satisfaction of the patients.It is worthy of clinical application.
