医院社区一体化延续护理对糖尿病患者自我血糖监测依从性的影响

摘要

目的:探讨医院社区一体化延续护理对糖尿病患者自我血糖监测依从性的影响.方法:选取2015年6~12月入住内分泌科的糖尿病患者138例,按随机数字表法分为观察组和对照组,除去失访者,最终纳入资料观察组66例,对照组69例,对照组住院期间给予常规血糖监测指导教育;观察组实施医院社区一体化延续护理进行护理干预.比较两组患者的自我血糖监测依从性、血糖监测频率、对血糖监测知识的掌握程度及出院后不同时间段的血糖值和糖化血红蛋白值.结果:观察组患者的自我血糖监测依从性得分、血糖监测频率、对血糖监测相关知识的掌握程度明显优于对照组,差异有统计学意义(P<0.05);1年后的空腹血糖、餐后2 h血糖和糖化血红蛋白值较对照组控制更佳,差异有统计学意义(P<0.05).结论:对糖尿病患者实施医院社区一体化延续护理有助于提高患者的血糖监测依从性,能更好的控制血糖值.%Objective:To discuss the influence of hospital and community integrated continued nursing on self-blood glucose monitoring compliance of diabetes patients.Methods:Selected 138 diabetes patients admitted in endocrinology department of our hospital from June to December 2015, and they were divided into observation group and control group according to random number method, upon exclusion of those losing follow-up visit, finally 66 patients in the observation group and 69 patients in the control group were included, and during hospitalization, conventional blood glucose monitoring guidance education was given to the patients in the control group, while hospital and community integrated continued nursing was given to the patients in the observation group for nursing intervention.Patients in the two groups were compared in self-blood glucose monitoring compliance, blood glucose monitoring frequency, mastering of blood glucose monitoring knowledge and blood glucose and glycosylated hemoglobin values in different periods after discharge.Results:The score of self-blood glucose monitoring compliance, blood glucose monitoring frequency and mastering of blood glucose monitoring related knowledge of patients in the observation group were superior to those of patients in the control group, and the difference was statistically significant (P<0.05);the fasting blood glucose 1 year later, blood glucose and glycosylated hemoglobin values 2h after meal of patients in the observation group were controlled better than those of patients in the control group, and the difference was statistically significant (P<0.05).Conclusion:The implementation of hospital and community integrated continued nursing for diabetes patients could improve the patients' blood glucose monitoring compliance to control the blood glucose value better.
