TSGF与CYFRA21-1联合检测对宫颈鳞癌患者的临床意义

Objective It is to approach the value of combined detection of serum tumor specific growth factor ( TSGF ) and cytokeratin 19 fragment antigen 21 - 1 ( CYFRA21 - 1 ) in the diagnosis of cervix squamous carcinoma.Methods The blood of 85 patients with cervix squamous carcinoma were collected to determine the content of TSGF with biochemical colorimetry and to detect the content of CYFRA21 - 1 with chemiluminescence immunoassay.Results With the increase of clinical stage of cervical cancer, the serum contents of TSGF and CYFRA21 - 1 both heightened ( both P ＜ 0.01 ).The positive rates of TSGF and CYFRA21 - 1 both increased with the increase of clinical stage ( both P ＜ 0.01 ).The serum levels of TSGF and CYFRA21 - 1 both had not obvious relationship with the differentiation degree of cervix squamous carcinoma ( both P ＞ 0.05 ).The sensitivity of the detection of TSGF and CYFRA21 - 1 in the diagnosis of cervical cancer were 53％ and 39％ respectively , and there was no significant difference ( P ＞ 0.05 ).The sensitivity of combined detection of TSGF and CYFRA21 - 1 in the diagnosis of cervical cancer was 72％ , compared with single marker of TSGF or CYFRA21 - 1 , there was both significant differences ( P ＜ 0.05 or 0.01 ).Conclusion Separate detection of the serum contents of TSGF or CYFRA21 - 1 both has significance on the diagnosis of cervix squamous carcinoma.Combined detection can heighten the diagnostic rate of cervix squamous carcinoma and the sensitivity and the accuracy of diagnosis.%目的 探讨联合检测血清恶性肿瘤特异性生长因子(TSGF)、细胞角蛋白19片段抗原21-1(CYFRA21-1)在宫颈鳞癌诊断中的价值.方法 采集85例宫颈鳞癌患者的血液,利用生化比色法测定TSGF的含量,利用化学发光免疫法检测CYFRA21-1的含量.结果 随宫颈癌临床分期的增加,血清TSGF和CYFRA21-1含量均升高(P均<0.01).同样发现TSGF和CYFRA21-1的阳性率随着临床分期增加而增加(P均<0.01).血清TSGF和CYFRA21-1血清水平与宫颈鳞癌分化程度均无明显的相关性(P均>0.05).血清TSGF和CYFRA21-1检测用于宫颈癌诊断的敏感性分别为53%和39%,两者比较无显著性差异(P>0.05);而CYFRA21-1与TSGF联合检测诊断宫颈癌的敏感度达到72%,与单一TSGF或CYFRA21-1指标相比均有显著性差异(P<0.05或0.01).结论 单独检测血清TSGF和CYFRA21-1含量对宫颈鳞癌有意义,联合检测能提高宫颈鳞癌的诊断率,提高诊断的敏感性和准确性.
