Objective It is to observe the effect of Tongmai Yiqi Chinese medicine combined with acupuncture on elderly lower extremity atherosclerosis and on the levels of ankle brachial index (ABI) and brachial-ankle pulse wave velocity (baPWV).Methods 80 cases of elderly patients with lower extremity atherosclerosis disease were randomly divided into two groups,the control group (40 cases) were treated with simvastatin combined with aspirin,the observation group (40 cases)were treated with Tongmai Yiqi Chinese medicine combined with acupuncture on the basis of treatment in control group.The changes of TCM syndrome scores,blood fat index,inflammatory cytokines,femoral artery intima-media thickness (IMT),ABI and baPWV before and after treatment were observed and the curative effects were evaluated in both groups.Results After treatment,TCM syndrome scores,the levels of TG,TC,LDL-C,CRP,D-dimer,femoral artery IMT and baPWV were significantly decreased(P ＜ 0.05),that of HDL-C and ABI were significantly increased in both groups(P ＜ 0.05),and the improvements of these indexes in observation group were better than those in control group (P ＜ 0.05).The total effective rate of observation group was significantly higher than that in control group (P ＜ 0.05).Conclusion Tongmai Yiqi Chinese medicine combined with acupuncture can significantly alleviate symptoms of lower limbs and whole body,improve blood lipid levels,inhibit the body's inflammatory response,decrease the thickness of femoral artery,and helps to regulate ABI and baPWV level in the elderly patients with lower extremity atherosclerosis.%目的 观察通脉益气中药合针灸治疗老年下肢动脉粥样硬化性疾病疗效及对ABI、baPWV水平的影响.方法 将80例老年下肢动脉粥样硬化性疾病患者随机分为2组,对照组40例给予辛伐他汀联合阿司匹林治疗,观察组40例在此基础上加用通脉益气中药合针灸治疗,观察治疗前后中医证候积分、血脂指标水平、炎性细胞因子水平、股动脉内-中膜厚度、ABI及baPWV水平,统计2组临床疗效.结果 2组治疗后中医证候积分,TG、TC、LDL-C、CRP、D-二聚体、baPWV水平及股动脉内-中膜厚度均显著降低(P均＜0.05),HDL-C和ABI水平均显著提高(P均＜0.05);且观察组治疗后各项指标水平改善情况均显著优于对照组(P均＜0.05);观察组治疗总有效率显著高于对照组(P＜0.05).结论 通脉益气中药合针灸治疗老年下肢动脉粥样硬化性疾病可显著缓解下肢及全身症状,改善血脂水平,抑制机体炎症反应,降低股动脉管腔厚度,且有助于调节ABI和baPWV水平.
