Objective It is to observe the effect of Qufeng Tongluo Yishen Chinese medicine on degenerative knee osteoarthritis of Shen deficiency blood stasis syndrome and effect on serum cartilage oligomeric matrix protein (COMP) and matrix metalloproteinases-3 (MMP-3).Methods 140 cases of patients with degenerative knee osteoarthritis of kidney deficiency and blood stasis syndrome were randomly divided into 2 groups,the control group (n =70) were treated with glucosamine hydrochloride (0.48 g/time,3 times/d) and celecoxib (0.2 g/time,once/d) by oral administration,the observation group (n =70) was treated with Yufeng Tongluo Yishen Chinese medicine on the basis of treatment in control group.Both groups were treated for 8 weeks.The changes of traditional Chinese medicine syndrome scores,osteoarthritis index (WOMAC) and the measurement of quality of life scale profile score (QOL-BREF) and serum levels of COMP,MMP-3 before and after treatment were observed,the recent curative effect and occurrence of side effects were compared in both groups.Results The recent total effective rate in observation group was significantly higher than that in control group (P ＜ 0.05);The scores of joint pain,joint swelling,bad joint action,joint adverse rubs,lumbar debility,dark purple tongue and pulse heavy fine after treatment were significantly lower than that before treatment in both groups(P ＜ 0.05),and the decrease of these scores in observation group were more significantly than that in the control group (P ＜ 0.05);WOMAC scores and QOL-BREF scores after treatment were significantly better while the levels of COMP and MMP-3 were significantly lower than that before treatment in both groups(P ＜ 0.05),the improvements of these indexes in observation group were significantly better than that in control group (P ＜ 0.05);There was no significant difference in Incidence of adverse reactions between the 2 groups (P ＞ 0.05).Conclusion Qufeng Tongluo Yishen Chinese medicine can effectively alleviate associated symptoms and signs,promote joint function recovery,improve quality of life,down-regulate the levels of COMP and MMP-3 with good safety in the treatment of degenerative knee osteoarthritis of Shen deficiency blood stasis syndrome.%目的 观察祛风通络益肾中药治疗退行性膝骨性关节炎肾虚血瘀证疗效及对血清软骨寡聚基质蛋白(COMP)、基质金属蛋白酶-3(MMP-3)水平的影响.方法 将140例退行性膝骨性关节炎肾虚血瘀证患者随机分为2组,对照组70例给予盐酸氨基葡萄糖(0.48 g/次,3次/d)和塞来昔布(0.2g/次,1次/d)口服;观察组70例在此基础上加用祛风通络益肾中药治疗.2组治疗时间均为8周.观察2组治疗前后中医证候积分、骨关节炎指数(WOMAC)、生存质量测定量表简表评分(QOL-BREF)及血清COMP、MMP-3水平变化情况,统计2组近期疗效及不良反应发生情况.结果 观察组近期总有效率显著高于对照组(P＜0.05);2组治疗后关节疼痛、关节肿胀、关节活动不利、关节摩擦音、腰膝酸软、舌质紫暗及脉沉细积分均显著低于治疗前(P均＜0.05),且观察组治疗后各项积分均显著低于对照组(P均＜0.05);2组治疗后WOMAC评分和QOL-BREF评分均显著优于治疗前(P均＜0.05),COMP和MMP-3水平均显著低于治疗前(P均＜0.05),且观察组各项指标改善情况均显著优于对照组(P均＜0.05);2组不良反应发生率比较差异无统计学意义(P＞0.05).结论 祛风通络益肾中药治疗退行性膝骨性关节炎肾虚血瘀证可有效缓解相关症状体征,促进关节功能恢复,改善日常生活质量,下调COMP和MMP-3水平,且安全性好.
