超声乳化人工晶体植入联合房角分离术治疗白内障合并闭角型青光眼48例疗效分析

摘要

Objective:To investigate the effect of phacoemulsification and intraocular lens implantation combined with goniosynechialysis in 48 patients with cataract and angle-closure glaucoma treatment effect.Method:48 cases of angle-closure glaucoma with cataract admitted patients into our hospital from August 2010 to August 2012 were selected as subjects.All of them were given phacoemulsification and intraocular lens implantation combine goniosynechialysis and follow-up for 6-12 months.The distribution of best correct visual acuity and the intraocular pressure situation of preoperative and postoperative,adverse reactions were recorded.Result:The difference of best correct visual acuity between preoperative and postoperative was statistically significant(P<0.05).The intraocular pressure of preoperative was(26.6±18.7)mm Hg and the intraocular pressure of postoperative was(11.5±4.1)mm Hg,the difference was statistically significant(P<0.05).9 cases of corneal edema after operation,after drug treatment were absorbed within a week.12 cases of iridocyclitis patients were absorbed after drug treatment.Conclusion:The author think that phacoemulsification and intraocular lens implantation combine goniosynechialysis has great effect on angle-closure glaucoma with cataract.It is safely and it is worth to be widely spread in clinic.%　　目的：探讨超声乳化人工晶体植入联合房角分离术对48例白内障合并闭角型青光眼患者的治疗效果。方法：2010年8月-2012年8月本院所收治的患者共48例，所有患者均接受白内障超声乳化人工晶体植入联合房角分离术。对患者进行6~12个月的随访。记录患者的术前、术后最佳矫正视力分布情况，术前术后眼压情况和不良反应发生情况。结果：术前、术后患者的最佳矫正视力差异有统计学意义（P<0.05）。患者术前用药后眼压为（26.6±18.7）mm Hg，术后眼压为（11.5±4.1）mm Hg，差异有统计学意义（P<0.05）。术后角膜水肿9例，经过药物治疗均在1周之内吸收；虹膜炎症反应12例，经过药物治疗后炎症渗出物吸收。结论：超声乳化人工晶体植入联合房角分离术对白内障合并闭角型青光眼的疗效显著，安全性高，值得在今后临床当中推广使用。
