腹腔镜胆囊切除术治疗急性胆囊炎患者的临床效果

摘要

目的：探讨腹腔镜胆囊切除术（LC）治疗急性胆囊炎患者的临床疗效。方法选取2012年4月至2015年4月于医院行LC治疗的150例急性胆囊炎患者，通过气管插管行全身麻醉，探查腹腔后根据操作难度实施4孔法或3孔法手术治疗，观察结局。结果顺利完成LC手术的有138例，手术时间平均（72.5±4.6）min。中转开腹者有12例，术后8例发生胆漏，采取保守治疗措施后痊愈。患者平均住院时间（7.6±1.2）d，均痊愈出院。1年随访期间没有出现胆源性胰腺炎、胆道损伤等并发症。结论采用LC治疗急性胆囊炎可取得良好的手术效果，并发症少、安全性高、预后效果好，值得推广。%ObjectiveTo investigate the clinical effect of laparoscopic cholecystectomy (LC) in the treatment of acute cholecystitis. Methods 150 cases of acute cholecystitis patients who accepted LC treatment in the hospital from 2012 April to 2015 April were selected. Through the trachea intubation general anesthesia, after the exploration of the abdominal cavity according to the operation dificulty of the implementation of the 4 hole method or 3 hole method of surgery treatment, observation of the outcome.Results 138 cases were successfuly completed LC, the average operation time was (72.5±4.6) min. There were 12 cases of conversion to open surgery, 8 cases of postoperative bile leakage, take the conservative treatment after recovery. The average length of stay (7.6±1.2) d in the patients were al recovered and discharged. No complications such as biliary pancreatitis and bile duct injury occurred during the folow-up period of 1 years.Conclusion The use of LC in the treatment of acute cholecystitis can achieve good operation efect, less adverse reaction, high safety, good prognosis, it is worth promoting.
