移植后应用补胞汤及着床后应用温肾安胎对体外受精-胚胎移植妊娠结局的研究

摘要

[目的]观察移植后应用补肾填精、养血活血为治法的补胞汤及着床后应用温补肾阳、安养胎儿为治法的温肾安胎汤对体外受精-胚胎移植妊娠结局的影响。[方法]收集接受新鲜胚胎移植( in vitro fertilization and embryo transplantation，IVF-ET )治疗的不孕症患者120例，随机分为治疗组62例和对照组58例。两组均接受常规IVF-ET方案促排卵治疗，治疗组移植后加服中药补胞汤，持续至移植10～12d后检测血HCG，并比较两组着床率。将着床成功患者43例随机分为着床后观察组21例和着床后治疗组22例，着床后治疗组加服中药温肾安胎汤至B超确定临床妊娠日。比较临床妊娠率、持续妊娠率及先兆流产症状评分。[结果]治疗组胚胎着床率高于对照组，差别有统计学意义（P<0.05）；两组临床妊娠率、持续妊娠率比较差异无统计学意义（P>0.05）；着床后治疗组先兆流产症候群积分低于着床后观察组，差异有统计学意义（P<0.05）。[结论]移植后应用补肾填精、养血活血为治法的补胞汤及着床后应用温补肾阳、安养胎儿为治法的温肾安胎汤能提高体外受精-胚胎移植的着床率，降低临床妊娠患者先兆流产症候群积分，改善妊娠结局。%Objective] To study the pregnancy outcomes of IVF-ET by using the Bu-Bao Tang with the effect of nourishing the kidney and replenishing essence, invigorating and activating the blood and using the Wen-Shen-An-Tai Tang after embryo implantation with the effect of warming the kidney and calming the fetus. [Methods]Total y 120 infertility patients accepted the treatment of IVF-ET and were randomly divided into the treatment group of 62 patients and the control group of 58 patients. Both groups received conventional therapy of ovulation induction of IVF-ET. The treatment group received Bu-Bao Tang for 10~12d by the day of detecting the HCG. The successful patients of implantation were randomly divided into observation group and the treatment group after implantation. The treatment group after implantation received Wen-Shen-An-Tai Tang by the day of determining the clinical pregnancy day by the sonography. The implantation rate, clinical pregnancy rate, ongoing pregnancy rate and the symptom scores of threatened abortion were compared. [Results]The implantation rate of the treatment group was higher than the control group, showing statistical difference between the two groups. There were no statistical differences of the clinical pregnancy rate and ongoing pregnancy rate between the two groups. The symptom scores of threatened abortion of the treatment group was lower than the pregnancy obseruation group, showing statistical difference between the two groups. [Conclusion ]The therapy of using the Bu-Bao Tang with the effect of nourishing the kidney and replenishing essence, invigorating and activating the blood after IVF-ET and using the Wen-Shen-An-Tai Tang with the effect of warming the kidney and calming the fetus after embryo implantation can improve the implantation rate, lower symptom scores of threatened abortion, and improve the pregnancy outcomes.
