腹针治疗妇科腹部手术患者术后粘连性腹痛临床效果分析

摘要

Objective To observe the clinical effect of abdominal acupuncture in treating ankylenteron abdominal pain after gynecological abdominal surgery.Methods Fifty-six gynecologic neoplasms patients with ankylenteron abdominal pain after abdominal surgery in Guangdong Provincial Hospital of Traditional Chinese Medicine from November 2015 to October 2016 were selected and divided into control group(n =28) and observation group(n =28) according to the treatment method.Two patients withdrew from the study because of occuring fever or diarrhea;Fifty-four patients were enroll in this study finally,with 27 patients in each group.The patients in control group were treated with oral traditional Chinese medicine according to syndrome differentia tion and sham abdominal acupuncture therapy;the patients in observation group were treated with oral traditional Chinese medicine according to syndrome differentiation and abdominal acupuncture therapy.The treatment procedure was blind for the pa tients.All patients were treated for 3 days.Before and after treatment,the degree of abdominal pain and abdominal distension symptoms of patients was evaluated by visual analogue scale.Besides,the white blood cell(WBC) counts and C-reactive protein (CRP) levels were detected and compared between the two groups before and after the treatment.Results There were no statis tic difference in the scores of abdominal distention and abdominal pain of patients before treatment between the two groups (P ＞0.05).The scores of abdominal distention and abdominal pain of patients after treatment were significantly lower than those before treatment in the two groups(P ＜0.05);and the scores of abdominal distention and abdominal pain of patients in observation group were significantly lower than those in the control group after treatment (P ＜ 0.05).There were no statistic difference in WBC counts and CRP levels of patients between the two groups before treatment(P ＞ 0.05).The WBC counts and CRP levels of patients after treatment were significantly lower than those before treatment (P ＜ 0.05) in the two groups;but there was no statistic difference in the WBC counts and CRP levels of patients between the two groups after treatment (P ＞ 0.05).Conclusion Abdominal acupuncture can effectively improve abdominal pain and abdominal distention symptoms in patients with ankylenteron abdominal pain after gynecological abdominal surgery.But its effect on the levels of WBC counts and CRP is not certain.%目的 观察腹针治疗妇科腹部手术患者术后粘连性腹痛的临床效果.方法 选择2015年11月至2016年10月在广东省中医院妇科因妇科良性肿瘤行经腹或经腹腔镜择期手术治疗且术后符合粘连性腹痛诊断标准的患者56例,根据治疗方法分为对照组(n=28)和观察组(n=28),治疗过程中因发热、腹泻退出2例,最终每组入组27例,对照组患者采用术后辨证口服中药+假腹针治疗,观察组患者采用术后辨证口服中药+腹针治疗.患者接受治疗过程采用盲法,共治疗3d.治疗前、后采用视觉模拟评分法对2组患者腹痛及腹胀症状进行评分,检测患者血中白细胞计数和C-反应蛋白水平并进行比较.结果 2组患者治疗前腹胀、腹痛评分比较差异均无统计学意义(P＞0.05);治疗后2组患者腹胀、腹痛评分较治疗前显著下降,且观察组患者较对照组下降更显著(P＜0.05).2组患者治疗前血中白细胞计数和C反应蛋白水平比较差异均无统计学意义(P＞0.05);治疗后2组患者血中白细胞计数和C反应蛋白水平均较治疗前显著下降(P＜0.05),但2组患者之间治疗后血中白细胞计数和C反应蛋白水平比较差异均无统计学意义(P＞0.05).结论 腹针治疗可有效改善妇科腹部手术患者术后粘连性腹痛患者的腹痛、腹胀症状,而在术后降低白细胞计数和C反应蛋白水平方面其作用尚不确切.
