单点注射A型肉毒素矫正咬肌肥大的作用评估

摘要

目的 单点法注射A型肉毒素矫正咬肌肥大的作用效果观察. 方法 选择2016-01 ～2017-01于我科室就诊的咬肌肥大患者45例,其中15例矫正方法为单点法注射A型肉毒素,余30例为多点法注射.从注射前后正面照片、B超测量咬肌厚度、患者痛觉评价及满意度随访三个方面对比两种注射方法的优缺点. 结果 注射前后正面照片提示两种方法对矫正咬肌肥大均有效,且两种注射方法B超测得随访第1,3,6个月的咬肌厚度与注射前相比均明显变小.但同一随访时期两种注射方法所得的数据间没有明显的差距.单点注射组痛觉评分1.4分,“非常满意”患者占97％;多点注射法痛觉评分5.3分,“非常满意”患者占70％. 结论 单点注射A型肉毒素矫正咬肌肥大操作简单,作用效果明显,值得推广和应用.%Objective To observe the effect of single point injection of botulinum toxin type A in treatment of masseter hypertrophy.Methods From January 2016 to January 2017,45 patients with masseter hypertrophy in our department were chosen,of which 15 patients were treated by the single point injection method,the other 30 patients were treated by the multiple point injection method.The advantages and disadvantages of the two injection methods were compared from the following three aspects:standardized photographs before and after injection,the thickness of masseter muscle before and after injection by ultrasound,and patients' pain evaluation and satisfaction.Results Two methods were both effective for masseter muscle hypertrophy according to photograph before and after injection.The thickness of masseter muscle evaluated by B-mode ultrasound was significantly reduced at 1,3,6 month after injection,but there was no significant difference between the two methods at the same follow-up time.In single point injection group,the pain score was 1.4,and the "very satisfied" patients accounted for 97％.In the multiple point injection method group,the pain score was 5.3,and "very satisfied" patients accounted for 70％.Conclusion Single point injection of botulinum toxin type A for masseter hypertrophy is effective and simple to operate.
