目的 探讨并分析行为转变理论在慢性心力衰竭患者自我护理与疾病感知中的应用效果.方法 选取2014年2月至2015年2月在我院接诊的108例患有慢性心力衰竭的患者,随机的分为观察组和治疗组各54例,观察组的患者给予常规护理,治疗组的患者在常规护理的基础上给予行为转变的自我护理干预,观察两组患者治疗的临床效果. 结果 入组时两组患者自我护理评估、疾病感知水平、心衰知识均及生活质量无显著性差异(P>0.05);在出院后的6个月及12个月的自我护理评估与疾病的感知得分比较中,治疗组的自我护理得分显著低于观察组(P<0.05),治疗组的感应水平得分显著高于观察组(P<0.05),治疗组的生活质量和心衰知识得分显著高于观察组(P<0.05). 结论 行为转变理论模式在慢性心力衰竭中的应用,提高了患者的自我护理和疾病感知水平,提高了慢性心力衰竭患者的自我护理能力及感知水平,改善生活质量,降低在住院率,可在临床上进行推广应用.%Objective To study and analyze the application effect of transtheoretical model of behavior in the self-care and Disease awareness level of patients with chronic heart failure.Methods Selected 108 cases with chronic heart failure admitted in our hospital from February 2014 to February 2014, randomly divided them into observation group and treatment group, each group had 54 cases, the observation group was given with routine care, the treatment group was given with transtheoretical model of behavior in the self-care on the basis of conventional nursing care , the clinical effects of two groups were observed.Results The scores of self-care, disease awareness level and heart failure and quality of life ( QOL) of heart failure of two groups had no significant differences at the time of enrollment(P>0.05);6 and 12 months after discharge from hospital, the scores of self-care of the treatment group was significantly lower than group ( P <0.05 ) , disease awareness level of the treatment group were significantly higher than in observation group ( P <0.05 ) , quality of life scores and knowledge of heart failure were significantly higher than in observation group in the treatment group ( P<0.05) . Conclusions The application of transtheoretical model of behavior on the chronic heart failure can improve the patient's level of self-care and disease awareness, improve the self-care ability of the patients with chronic heart failure and the level of awareness, improve the quality of life, reduce the hospitalization rates, which can be applied in clinic.
