多巴酚丁胺治疗缓慢心率心力衰竭的疗效及安全性

摘要

目的 观察多巴酚丁胺在治疗缓慢心率心力衰竭中的疗效及安全性. 方法 选择心率<55次/分缓慢心率心力衰竭患者56例,在常规利尿、扩血管等基础治疗上,静脉泵入多巴酚丁胺2~5μg/kg· min,1周为1个疗程,比较治疗前后临床症状、左室射血分数、BNP水平的变化,评估其临床有效性和安全性. 结果 治疗1周后,心衰症状缓解51例,总有效率91%,左室射血分数由(33.2 ±4.7)%升至(48.5 ±5.1)%,BNP水平由(1476 ±215.8) pg/ml降至(853 ±106.2) pg/ml,差异均有统计学意义( t=9. 616、11.291,P<0.001). 本组患者在使用多巴酚丁胺过程中未发现明显副作用. 结论 对于缓慢心率心力衰竭患者在常规抗心衰药物治疗的基础上联合应用多巴酚丁胺安全有效,不良反应少.%Objective To observe the efficacy and safety of dobutamine in the treatment of slow heart rate in heart failure.Methods 56 cases of heart failure patients whose heart rate less than 55 BPM were enrolled in the study, on the basis of conventional diuretic and vasodilator therapy, venous pump into the dobutamine at the dose of 2-5μg/kg/min, one week as a course of treatment, compared the clinical symptoms, left ventricular ejection fraction ( LVEF) , BNP level changes to assess the clinical effectiveness and safety.Results One week after the treatment, 51 cases of heart failure symptoms relieved, the total effective rate was 91%, left ventricular ejection fraction raised to (4.7 ±5.1)%, BNP level decreased to (853 ±106.2) pg/ml, the differences were statistically significant (t=9.616, 11.291,P<0.001).This group of patients without obvious side effects in the use of dobutamine in the process.Conclusions For the patients with slow heart rate heart failure, routine anti-heart failure drugs combined with dobutamine is safe and effective, with less adverse reactions.
