多层螺旋CT薄层扫描和后处理重建在眼外伤致眼眶壁骨折诊断中的应用

         

摘要

Objective To explore the value of multi-slice spiral CT scan and reconstruction in the diagnosis of eye trauma-induced orbital wall fracture. Methods Thin-slice CT scan was performed with 16-slice spiral CT scanner in 36 cases of ocular trauma-induced orbital wall fracture. Volume rendering (VR), shaded surface display (SSD) and multi-planar reconstruction (MPR) were made at the Extended Brilliance Workspace (EBW,V4. 5. 2. 4031) workstation. Results A-mong the 36 patients, burst fractures were found in 17, direct fractures in 11 and compound fractures in 8. Of the 45 fractures, medial orbital wall fractures were found in 22, orbital floor fractures in 8, supraorbital wall fractures in 8, lateral orbital wall fractures in 5, and orbital apex and optic canal fractures in 2. In addition, single-wall fractures were found in 17 patients, double-wall fractures in 11, three-wall fractures in 6 cases, and four-wall fractures in 2. Conclusion Multi-slice spiral CT scan and reconstruction can improve the diagnosis of orbital wall fracture and provide a more intuitive understanding of the actual size, shape, location of orbital wall fracture and its anatomical relationship to surrounding structures in three-dimensional space. Therefore,multi-slice spiral CT scan and reconstruction may provide a reliable diagnostic imaging basis for clinical treatment of orbital wall fracture.%目的 探讨多层螺旋CT薄层扫描和后处理重建对眼外伤致眼眶壁骨折的诊断价值.方法 对36例眼外伤致眼眶壁骨折患者采用16层螺旋CT机进行薄层扫描,于EBW(V4.5.2.4031)工作站行各种后处理重建[采用容积再现技术(VR)、表面遮盖法(SSD)和多平面重建(MPR)技术进行重建].结果 36例患者中,骨折类型:爆裂骨折17例,直接骨折11例,复合骨折8例;骨折部位:有45处骨折,其中眶内侧壁22处,眶下壁8处,眶上壁8处,眶外侧壁5处,眶尖及视神经管骨折2处;骨折范围:单壁骨折17例,双壁骨折11例,三壁骨折6例,四壁骨折2例.结论 多层螺旋CT薄层扫描和后处理重建可提高对眼外伤致眼眶壁骨折的诊断水平,较直观地了解眼眶壁骨折在三维立体空间的实际大小、形态、位置及周围结构的解剖关系,为临床选择治疗方案提供可靠的影像诊断依据.
