静脉注射三磷酸腺苷与普罗帕酮治疗阵发性室上性心动过速疗效比较的Meta分析

摘要

目的：探讨静脉注射三磷酸腺苷与普罗帕酮治疗阵发性室上性心动过速（PSVT）的疗效及安全性。方法计算机检索PubMed和 Elsevier、CNKI、万方、维普和中国生物医学文献数据库等数据库，搜索有关静脉注射三磷酸腺苷与普罗帕酮治疗PSVT的疗效及安全性的文献，检索时间为建库至2016年1月，根据纳入标准对文献进行筛选和质量评价，采用 RevMan5．3软件对数据进行 Meta分析。结果共纳入16篇文献，结果显示静脉注射三磷酸腺苷治疗PSVT的复律有效率显著高于普罗帕酮（OR＝2．17，95％CI为1．52～3．11，P＜0．0001），复律时间也较普罗帕酮明显缩短（SMD＝－2．86，95％CI为－3．32～－2．41，P＜0．00001），但不良反应发生率显著高于普罗帕酮（OR＝2．80，95％CI为1．78～4．40，P＜0．00001）。结论静脉注射三磷酸腺苷治疗 PSVT的复律有效率及复律时间方面优于普罗帕酮，但不良反应发生率高于普罗帕酮。对于不同的患者，应结合患者的不同情况，来选择不同的药物。%ABSTRACT:Objective To evaluate the efficacy and safety of intravenous injection of triphosaden or propafenone in the treatment of paroxysmal supraventricular tachycardia (PSVT).Methods We researched PubMed,Elsevier,CNKI,Wanfang,VIP and CBM databases from inception to Jan-uary 2016 to collect the literature about the efficacy and safety of intravenous injection of tripho-saden or propafenone in the treatment of PSVT.The studies were screened and evaluated accord-ing to the inclusion criteria.The meta-analysis was performed by using RevMan5.3.Results A total of 1 6 studies were included.The meta-analysis showed that,compared with intravenous propafenone injection,triphosaden increased the effective cardioversion rate(OR=2.17,95% CI:1.52-3.11,P<0.000 1),shortened the time to cardioversion(SMD=-2.86,95% CI:-3.32--2.41,P<0.000 01),but increased the incidence of adverse reactions(OR=2.80,95%CI:1.78-4.40,P<0.000 01).Conclusion Compared with propafenone,intravenous triphosaden injection can increase effective cardioversion rate,shorten cardioversion time,but increase adverse reactions in patients with PSVT.Therefore,drugs should be chosen according to different conditions of PS-VT patients.
