ESR-30型全自动血沉分析仪性能评价及健康人群血红细胞沉降率生物参考区间的建立

         

摘要

目的 用ESR-30型全自动血沉分析仪检测健康人群红细胞沉降率,建立该方法的红细胞沉降率生物参考区间,为临床提供更准确的诊断参考依据,并与血红细胞沉降率的经典方法(魏氏法)进行对比试验,评价两者结果的相关性.方法 收集医院体检中心经体检医生全面检查未发现有引起血红细胞沉降率改变的各种非生理性疾病的人群共372例,其中男165例,女207例,<10岁52例,10~30岁155例,30~50岁123例和>50岁42例,用ESR-30型全自动血沉仪测定其血红细胞沉降率,对数据进行统计学分析与比较,并随机抽取临床标本54例,其中男24例,女30例,年龄17岁~62岁,用ESR-30型和经典方法(魏氏法)同时对比测定并统计分析两者测定结果的相关性.结果 ESR-30型全自动血沉仪检测健康人群血红细胞沉降率的生物参考区间为:男性小儿组(<10岁)为2.0~13.2 mm/h,男性青年组(10~30岁)为2.0~24.9 mm/h,男性中年组(30~50岁)为2.0~37.2 mm/h,男性老年组(>50岁)为2.0~45.9 mm/h,女性小儿组(<10岁)为2.0~13.6 mm/h,女性青年组(10~30岁)为2.0~28.7 mm/h,女性中年组(30~50岁)为2.0~43.6 mm/h,女性老年组(>50岁)为2.0~48.3 mm/h,除小儿组外其它不同性别、不同年龄组间ESR-30型全自动血沉仪测定的血红细胞沉降率差异有统计学意义(P<0.05).ESR-30型全自动血沉仪测定结果与魏氏法比较差异无统计学意义(P>0.05),两种方法的相关性良好(r=0.814).结论 ESR-30型全自动血沉仪是一种检测红细胞沉降率的新方法,快速、准确、可靠,结果与魏氏法结果无差异,值得临床推广使用.不同年龄、不同性别血红细胞沉降率有差异,不同人群应使用不同的参考范围.因此,必须建立该方法的不同健康人群血红细胞沉降率生物参考区间,为临床诊断提供更准确的诊断参考依据.%Objective Use ESR-30 fully automatic analysis of erythrocyte sedimentation rate of detection healthy people to establish the method of blood erythrocyte sedimentation rate of biological reference interval to provide a more accurate diagnosis for clinical reference,and erythrocyte sedimentation rate and blood of classics (WeiKjeidahl method) comparison tests, and evaluation of the results between the two. Methods Collected 372 cases from the physical examination of the Hospital Medical Center,a comprehensive examination by the medical examiner found no cause blood erythrocyte sedimentation rate changes in a variety of non-physiological disease of the crowd,and 165 cases of male,207 cases of female,<10 years old:52 cases,the age of 10~30:155 cases,the age of 30~50:123 cases and >50 years o1d:42 cases. Their blood erythrocyte sedimentation rates were determined with ESR-30 automatic ESR instrument and did the statistical analysis and comparison of the data. And randomly selected 54 cases of clinical specimens, 24 cases of male and 30 cases of female, the age of 17~62 years old. Analysed the correlation of two measurement results between the ESR-30 and classical methods (Wilcoxon method). Results ESR-30 automatic ESR was detected in healthy population of blood erythrocyte sedimentation rate of the biological reference interval for the group of young men (10 to 30 years old) :ma1e pediatric group (<10 years) was 2. 0 to 13. 2 mm/h,2. 0~24. 9 mm/h,ma1e middle-aged group (30~50 years) was 2. 0~ 37. 2 mm/h,ma1e old age group (>50 years old) to 2. 0~45. 9 mm/h,the female children (<10 years) was 2. 0~13. 6 mm/h,the young female group (10~30 years) 2. 0~28. 7 mm/h,the female middle-aged group (30~50 years) 2. 0~43. 6 mm/h,elderly women (>50 years) 2. 0 ~48. 3 mm/h,different gender, different age groups except pediatric group between the ESR-30 automatic ESR measured blood erythrocyte sedimentation rate difference was statistically significant (P<0. 05). ESR-30 automatic ESR measured results with the Wilcoxon method differences not statistically significant (P> 0. 05) ,the correlation of the two methods (r= 0. 814). Conclusion ESR-30 automatic ESR meter is a new method for detection of ESR,fast, accurate and reliable results with the Wilcoxon method,is worthy of clinical use. Different ages and gender of blood erythrocyte sedimentation rate differences, different groups use different reference ranges. Therefore,must establish the different methods of healthy people blood erythrocyte sedimentation rate of biological reference interval, that the clinical diagnosis to provide a more accurate diagnosis reference.
