目的：探究高剖宫产率的原因，寻找降低高剖宫产率的措施。方法：将我院2013年住院分娩的447例孕妇作为干预组，2012年住院分娩的525例产妇作为对照组。笔者根据孕产妇的一般情况、选择性剖宫产、顺产、助产、中转剖宫产及相关管理因素确定了200项指标构建的“孕妇分娩方式合理性评价体系”，对对照组产妇的病历资料进行回顾性分析，对干预组孕妇的住院分娩经过进行全程指导及干预。结果：我院孕妇剖宫产率由干预前的48．00％下降为36．02％，其中初产妇与经产妇剖宫产率分别由干预前的46．15％和51．34％下降为37．80％和33．68％（ P＜0．05），具有显著差异。结论：“孕妇分娩方式合理性评价体系”操作简单、实用，是降低剖宫产率重要措施之一。%Objective :Probing into the cause of the high rate of cesarean section ,looking for measures to reduce high cesarean section rate .Methods :Our hospital from 2013 to 447 cases of pregnant women in childbirth as intervention group ,2012 ,525 cases hospitalized childbirth puerpera as control group .By a has a subtropical high title of obstetrics and gynecology doctors by the author according to the general situation of maternal selective cesarean birth midwives transit cesarean delivery and related management factors determine 200 pregnant childbirth way rationality of indicators to construct evaluation system ,to control the medical records were retrospectively analyzed .The intervention group of pregnant women in hospital childbirth after full guidance and intervention .Results:Our pregnant women cesarean deliv‐ery rate reduced to 36 .02% ,from 48 .00% before intervention among first‐time mothers and multipara cesarean section rate from 46 .15% before intervention and 51 .34% respectively 37 .80% and 33 .68% (P< 0 .05) ,with significant difference .Conclusion:Pregnant childbirth way rationality evaluation system the operation is simple practical ,is one of the important measures to reduce cesarean section rate .
