裂隙式关节成形术、关节盘复位＋关节成形术治疗单侧颞下颌关节强直的长期疗效比较

摘要

Objective :To compare the long‐term curative effect of fissure type arthroplasty ,disc reduction+ arthro‐plasty in treatment of unilateral temporomandibular joint ankylosis .Methods :Select 100 cases of unilateral temporoman‐dibular joint ankylosis patients in our hospital from January 2009 to January 2012 as research object ,according to the order of admission were divided into two groups ,the control group underwent fracture type arthroplasty ,observation group with disc reduction + arthroplasty ,followed up for 1 to 2 .5 years ,compared with curative effect ,the opening degree and complications of two groups .Results:Postoperative follow‐up of 1 to 2 .5 years ,the total efficiency of the ob‐servation group 98 .0% ,compared differently in control group 84 .0% (P<0 .05) .In addition ,opening degree of two groups after 1 years[(3 .04 ± 0 .44)cm VS (2 .63 ± 0 .61)cm] ,2 years[(3 .03 ± 0 .37)cm VS (2 .41 ± 0 .74)cm] ,differ‐ence was statistically significant (P<0 .05) .Conclusion:Compared with the slit type arthroplasty ,articular disc arthro‐plasty can recover opening more obvious ,the long‐term effect is better ,can be used as an important treatment of unilat‐eral temporomandibular joint ankylosis .%目的：比较裂隙式关节成形术、关节盘复位联合关节成形术治疗单侧颞下颌关节强直的长期疗效。方法：以我院2009年1月－2012年1月收治的100例单侧颞下颌关节强直患者为研究对象，按照入院先后顺序将其分为两组，对照组患者行裂隙式关节成形术，观察组患者则行关节盘复位＋关节成形术，术后随访1～2．5年，比较两组疗效、开口度及并发症情况。结果：术后随访1～2．5年，观察组总有效率98．0％，与对照组的84．0％比较差异具有统计学意义（P＜0．05）。另外，两组患者术后1年[（3．04±0．44）cm VS （2．63±0．61）cm]、2年[（3．03±0．37）cm VS （2．41±0．74）cm]开口度比较差异有统计学意义（P＜0．05）。结论：相比裂隙式关节成形术，关节盘复位＋关节成形术能更明显的恢复患者开口度，远期疗效更佳，可作为单侧颞下颌关节强直治疗的重要手段。
