目的::探讨心肌梗塞后心腔内血流动力学的改变及其对左心室重构的影响。方法:利用CDFI联合PD能够较直观而准确地定量心腔内不同部位血流状态的技术,对22例OMI病人和20例正常人左心腔内不同部位血流状态及其速度分布进行了定性和半定量分析。结果:正常人心室内各部位收缩期血流间自流出道会聚,速度剖面平滑,无紊乱血流;而且心尖部、心室中尖部、左室流出道血流速度加速度递增,说明正常心室收缩无论在时相上,还是力度上均高度协调一致。结论:MI后左心室的重构使心腔内血流发生了严重紊乱;而心腔内血流的紊乱及压力不协调可能是左心室晚期重构的重要影响因素之一。这不仅为合理解释心室晚期重构增加了新内容;同时为临床延缓左室重构的治疗措施提供了新思路。%Objective:To explore the hemodynamic changes in cardiac chambers after myocardial infarction and its effects on left ventricular remodeling. Methods:Joint PD technology, the use of a CDFI can more intuitive and accurate quantitative heart cavity blood flow to different parts of the state, 22 cases of OMI patients and 20 cases of normal left heart cavity blood flow to different parts of the state and its velocity distribution has carried on the qualitative and semi-quantitative analysis. Results:Each part of the normal heart indoor systolic blood flow convergence between their debut, velocity profile is smooth,no disorder flow;and apex,ventricle tip,the left ventricular outflow blood flow speed in the acceleration, normal ventricular systolic phase, when no matter in which highly coordinated or strength. Conclusion:Left ventricular remodeling after MI resulted in serious disorder in the heart cavity blood flow;and heart cavity blood disorders and pressure mismatch may be one of the important influence factors of left ventricular late reconstruction. This not only for reasonable explanation late ven-tricular remodeling added new content. At the same time delay for clinical left ventricular remodelingtreatment provides a new train of thought.
