动态检测红细胞RDW-CV和血清NSE、EPO在新生儿缺氧缺血性脑病的临床意义

         

摘要

Objective:To investigate the clinical significance of red blood cell volume distribution width ( RDW - CV ) , serum neuron specific enolization enzyme ( NSE ) , and the dynamics of erythropoietin( EPO) dynamic detection in neonatal hypoxic ischemic encephalopathy( HIE) . Methods:82 cases of newborn hypoxic ischemic encephalopathy in 1 d,3 d,7 and 10 d were examined by elec-trochemical luminescence detection of serum NSE,the variation in content of EPO,and the red blood cells RDW-CV in each period. 40 healthy full-term newborns were selected as the control. Results:The levels of serum NSE,EPO(39. 5±6. 81 ng/L 126. 2±32. 5 mIU/mL)in HIE1 day born group were significantly higher than the control group ( 9 . 61±2 . 42 ng/L 、13 . 5±4 . 2 mIU/mL P<0 . 01 ) , and a drop in the 7 day,compared with the control group with significant difference(P<0. 05);RDW-CV in 1 day was significantly higher than control group ( 19 . 6±9 . 0 P<0 . 01 ) , in the 10 day significantly decreased,compared with the control group there was no significant difference(P>0. 05). Conclusion:The children with HIE have braindamage more severe,the higher the levels of NSE,EPO and RDW-CV,with the decreasing when the illness recovery;The detection of the three indicators are important for accurate understanding of brain damage.%目的:探讨红细胞体积分布宽度( RDW-CV)、血清神经元特异性烯醇化酶( NSE)、和促红细胞生成素(EPO)动态检测在新生儿缺氧缺血性脑病(HIE)中的临床意义。方法:电化学发光法检测82例缺氧缺血性脑病新生儿出生1 d、3 d、7 d和第10 d血清中NSE,EPO的含量变化,及各时段红细胞RDW-CV。选40例健康足月新生儿为对照。结果:HIE组出生1 d血清NSE、EPO的水平(39.5±6.81 ng/L 126.2±32.5 mIU/mL)明显高于对照组(9.61±2.42 ng/L 、13.5±4.2 mIU/mL P<0.01),第7 d时有所下降,与对照组比较有显著性差异(P<0.05);RDW-CV 在第1 d明显高于对照组(19.6±9.0 P<0.01),第10 d明显下降,与对照组比较无显著性差异(P>0.05)。结论:HIE患儿生后脑损伤越重,血NSE、EPO和RDW-CV越高,随病情恢复进行性降低;该三项指标的检测对准确了解脑损伤有重要意义。
