透刺法合耳穴压豆治疗老年髋部骨折术后疼痛的临床观察

摘要

Objective To observe the analgesia effect of penetration acupuncture therapy combined with auricular plaster therapy for the treatment of postoperative pain of hip fracture in senile patients.Methods Sixty aged hip fracture patients were equally randomized into treatment group and control group.The treatment group was treated with penetration acupuncture therapy and auricular plaster therapy,and the control group was given intramuscular injection of tramadol hydrochloride injection.Before treatment,and 30 minutes and 2,6,24 hours after treatment,pain rating index (PRI) was assessed by using the short-form McGill Pain Questionnaire (SF-MPQ),pain scoring was made according to Visual Analog Scale (VAS),and present pain intensity (PPI) was examined in both groups.The adversereaction was also evaluated after surgery.Results After treatment,the total scores of PRI,VAS scores and PPI scores were all decreased in both groups at various time points after treatment (P＜0.01 compared with those before treatment),and the differences of PRI total scores,VAS scores,and PPI scores 30 min and 6h after treatment were significant between the two groups (P＜0.05).The incidence of adverse reaction in the treatment group was lower than that in the control group (P＜0.05).Conclusion Penetration acupuncture therapy combined with auricular plaster therapy has similar effect to tramadol hydrochloride in relieving postoperative pain of hip fracture,and is rapid in starting analgesia effect but has less adverse reaction.%[目的]观察透刺法结合耳穴压豆治疗老年髋部骨折术后的镇痛效果.[方法]将60例髋部骨折术后患者随机分为治疗组和对照组各30例.治疗组采用透刺法结合耳穴压豆治疗髋部骨折术后疼痛,对照组采用盐酸曲马多针肌肉注射镇痛.2组患者于治疗前、治疗后30 min、2h、6h、24 h采用McGill疼痛问卷(SF-MPQ)进行疼痛分级指数(PRI)、疼痛视觉模拟尺(VAS)以及现时疼痛强度(PPI)评分,并观察术后镇痛的不良反应.[结果](1)治疗后2组各时点的PRI总分、VAS评分、PPI评分均较治疗前降低,差异均有统计学意义(P＜0.01).治疗后组间比较,治疗组的PRI总分、VAS评分、PPI评分在治疗后30 min、治疗后6h与对照组相比均有统计学差异(P＜0.05).(2)治疗组的不良反应发生率低于对照组(P＜0.05).[结论]透刺结合耳穴压豆应用于髋部骨折术后镇痛,能有效减轻髋部骨折术后疼痛程度,与盐酸曲马多比较,透剌结合耳穴压豆镇痛作用起效迅速,不良反应率少.
