利用血管同步计划控制增强CT扫描最佳时机的实验

     

摘要

目的摸索血管同步计划(SIVP)使用方法并探索其应用价值. 方法随机抽取30(男17,女13)例疑有肝、肾、胰疾病的患者,于平扫后利用SI VP软件行SIVP实验,观察靶平面腹主动脉内造影剂浓度变化. 结果腹主动脉造影剂浓度从无到有,迅速达高峰,然后随之快速下降. 每个患者腹主动脉的浓度变化可用时间密度曲线表示,从中可计算出每位患者最佳增强扫描时间,最后根据此时间进行实际扫描.30例中除2例因人为操作失误失败以外,其余28例均获满意效果. 提示SIVP实验与实际扫描有很好的一致性. 结论 CT增强最佳扫描时机受多种因素的影响,如血液动力学、造影注射部位、注射速率等,SIVP通过在实际扫描前注射少量造影剂并监测靶区造影剂浓度变化,确定不同个体最佳扫描时间,使获得最佳增强CT扫描图像成为可能,从而有助于改善CT诊断能力,值得进一步推广应用.%AIM To study the method of synchronous vas cular injection planning (SIVP) and evaluate its va lu e of clinical applications. METHODS Experiments of SIVP wer e performed at random in 30 cases suspected with hepatic, renal, pancreatic les ions after moncontrast CT scan. The changes of abdominal aorta in CT value we re observed. RESULTS Aorta rapidly shows increased density at first, then decreased quickly. The change of density in aorta can be de-scribed by time-density curve. The actual CT scans were performed according to the results of SIVP. 28 cases obtained a satisfactory results. CONCLUS ION Optimal scan ti ming varies according to hemodynamic parameters, the contrast injection site, th e rate of injection, et al. SIVP makes it possible to easily and accurately adjust the scan timing by injection of a small amount of contrast medium and detection of the arrival of contrast medium at the target region prior to ac tual image acquisiton. It is helpful in improving the diagnostic capabilities of CT scanning.
