扶元通痹汤配合臭氧注射对急性腰椎退行性骨关节炎病疗效、VAS评分、ODI指数、血清细胞因子的影响

摘要

Objective:To explore the effect of Fuyuan Tongbi Decoction combined with ozone injection on acute degenerative osteoarthritis of lumbar spine,visual analogue (VAS)score,oswestry dysfunction index (ODI)index and serum cytokines. Methods:106 cases of patients with acute degenerative osteoarthritis of lumbar spine were selected as the study objects,and were divided into the study group and the control group,according to random number table method,53 cases in each group.The control group were given ozone injection therapy,and the study group were given Fuyuan Tongbi Decoction on the basis of the control group,all treated for 2 weeks. TCM syn-drome scores,VAS score,ODI score and serum interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α),matrix metalloproteinase-3 (MMp-3),bone morphogenetic protein (BMp-7) and other cytokines were figured before treatment and 1 week and 2 weeks after treatment. Clinical efficacy was evaluated,and the recurrence within 1 year was observed in the two groups. Results:1 week and 2 weeks after treatment,TCM syndrome scores,VAS score and ODI score of both groups decreased gradually(P<0.05),and those in the study group were significantly lower than those in the control group (P<0.05). After treatment of 1 week and 2 weeks,levels of serum IL-6, TNF-α,MMp-3 and BMp-7 decreased gradually(P<0.05),and those in the study group were significantly lower than those in the control group (P<0.05).After 2 weeks treatment,total effective rate of the study group was sig-nificantly higher than that of the control group,and the recurrence rate in 1 year was lower than that in the control group(P<0.05).Conclusion:Fuyuan Tongbi Decoction combined with ozone injection on acute degenerative os-teoarthritis of lumbar spine has a certain effect,and can effectively improve patients'lumbar function,and regulate cytokine levels.%目的 观察扶元通痹汤配合臭氧注射治疗急性腰椎退行性骨关节炎病疗效及对其视觉模拟(VAS)评分、Oswestry功能障碍指数(ODI)、血清细胞因子的影响.方法 将患者106例按照随机数字表法分为研究组和对照组各53例,对照组给予臭氧注射治疗,研究组在对照组的基础上给予扶元通痹汤治疗,两组均持续治疗2周.统计两组治疗前、治疗1周、2周中医证候积分、VAS评分、ODI评分及血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、基质金属蛋白酶-3(MMp-3)、骨形态发生蛋白(BMp-7)等细胞因子水平,评价两组临床疗效,随访观察两组1年内复发情况.结果 治疗1周、2周后,两组中医证候积分、VAS评分、ODI评分均逐渐降低(P<0.05),且研究组显著低于对照组(P<0.05);治疗1周、2周后,两组血清IL-6、TNF-α、MMp-3、BMp-7水平均逐渐降低(P<0.05),且研究组显著低于对照组(P<0.05);治疗2周后研究组总有效率显著高于对照组,1年内复发率显著低于对照组(P<0.05).结论 扶元通痹汤配合臭氧注射治疗急性腰椎退行性骨关节炎病疗效肯定,可有效改善患者腰椎功能,调节细胞因子水平.
