3D ASL显示手运动功能区与附近病变关系的研究

     

摘要

目的 基于脑功能磁共振成像( fMRI)与术中皮层直接电刺激( direct current stimulation,DCS)技术,探索三维动脉自旋标记(3D-arterial spin labeling,3D-ASL)评估脑部疾病患者脑皮层手运动功能区与附近病变关系的价值.方法 给46例大脑皮层手运动功能区附近病变患者行ASL的运动态与静息态fMRI扫描,计算得到能够显示手运动功能区与附近病变关系的图像,并与术中DCS及fMRI结果相比较;评估3D ASL显示手运动功能区与附近病变关系的准确性.结果 本组46例患者的3D ASL减影图像上手运动功能区均能显示,表现为手运动功能区受累、分散、变小、信号强度降低、变形、移位或无改变等;并且与DCS和fMRI结果一致.结论 3D ASL显示的脑部疾病患者脑皮层手运动功能区与附近病变关系与DCS和fMRI的结果一致,有很好的临床应用价值.%To explore the values of arterial spin labeling technique (3D ASL) in assessing the relationship between hand motor function and adjacent lesions in patients with brain diseases based on functional magnetic resonance imaging ( fMRI) and intraoperative direct current stimulation(DCS).Method 46 patients with brain damages near the motor area of the hand were collected.All patients were scanned with 3D ASL task state and resting fMRI.The images showed the relationship between the motor area of the hand and the surrounding lesions were calculated .The accuracy of 3D ASL in displaying the relationship between hand motor function area and adjacent lesions was compared with the results of DCS during operation and fMRI.Results In this study, 46 cases of 3D ASL images of upper hand motor function areas could be displayed , which showed that the hand motor function areas were affected , dispersed, smaller, reduced signal intensity , deformation, displacement, no change, and so on.Moreover, the results were consistent with DCS and fMRI results.Conclusion 3D ASL shows that the relationship between hand motor function area and adjacent lesions in patients with brain diseases is consistent with the results of DCS and fMRI, which has good clinical application prospects.
