Objective To explore the influence factors of incision healing in elderly patients with lower limb fracture surgery,observe cognitive function of elderly patients and summarize the nursing counter-measures. Methods Thirty - two elderly diabetic patients with lower limb fractures were selected from January 2010 to January 2012 and treated with surgical therapy. Montreal Cognitive Assessment (MCA) was implemented before surgery to screen for patients with mild cognitive impairment (MCI),and then they were given targeted nursing. Control of blood glucose,diabetic education,dietary nursing and functional exercise were monitored particularly. Results Among 32 patients,30 patients'postoperative incisions were healed in the first stage without any infections or nursing complications,and 2 patients'incisions were healed in the second stage. Conclusion In elderly diabetic patients with fracture,the postoperative incision healing is slow and easy to be infected. Therefore it is very important to strictly control blood glucose,pay great attention to dietary nursing and improve the patients'abilities of self - management. Furthermore,health education according to different degrees of cognitive disorder and rehabilitation involving family members are the key factors to improve the incision healing after operation.%目的 探讨老年糖尿病患者下肢骨折术后影响切口愈合的原因,重点观察老年患者的认知功能,并总结护理对策.方法 2010年01月至2012年01月,对32例下肢骨折合并2型糖尿病的老年患者行手术治疗,术前应用蒙特利尔认知量表(MoCA)进行测评,筛查轻度认知功能障碍(MCI)患者,给予针对性护理,重点是监测控制血糖、糖尿病教育、饮食护理、功能锻炼.结果 本组32例患者术后切口30例一期愈合,无创口感染现象,无护理并发症发生；2例二期愈合.结论 老年糖尿病患者骨折后切口愈合缓慢,易感染,严格控制血糖、重视饮食护理、提高患者自我管理能力,特别是针对老年人不同程度的认知障碍给予健康教育、让家属参与到疾病的治疗康复中是术后促进切口愈合的关键.
