二甲双胍结合炔雌醇环丙孕酮片对多囊卵巢综合征妇女内分泌的影响

         

摘要

目的 探讨二甲双胍与炔雌醇环丙孕酮片(达英-35)联用对多囊卵巢综合征(PCSO)妇女内分泌的影响.方法 选取PCSO患者136例,平均分为对照组和观察组.对照组采用达英-35单独治疗,观察组采用二甲双胍和达英-35联合治疗,比较2组治疗前后的总体有效率、患者体内激素相关指标[促黄体生成素(LH)、促卵泡激素(FSH)、睾酮(TEST)、雌激素(E2)、性激素结合蛋白(SHBG)]、胰岛素抵抗情况[稳态胰岛素评价指数(HOMA-IR)、胰岛素分泌指数(HOMA-β)].结果 观察组总有效率为97.1%, 显著高于对照组的75.0% (P<0.01);观察组治疗后各激素相关指标LH、FSH、TEST、E2、SHBG均显著改善(P<0.01), 对照组治疗后激素相关指标LH、TEST、E2显著改善(P<0.01), 且观察组各激素相关指标均显著优于对照组(P<0.01);观察组治疗后HOMA-IR、HOMA-β显著降低(P<0.01), 且显著低于对照组(P<0.01).结论 二甲双胍联合炔雌醇环丙孕酮片可显著改善患者内分泌紊乱和胰岛素抵抗,提高整体治疗有效率.%Objective To observe effect of effect of metformin combined with Diane-35 on endocrine of patients with polycystic ovarian syndrome (PCOS).Methods A total of 136 cases with PCOS in our hospital were randomly divided into observation group and control group, 68 cases in each group.The control group was treated with Diane-35 only, while the observation group was treated with Diane-35 and metformin.The overall effective rate, hormone indicators [luteinizing hormone (LH), follicle-stimulating hormone (FSH), testosterone (TEST), estrogen (E2), sex hormone binding globulin (SHBG)], and insulin resistance condition [homeostasis model assessment of insulin resistance (HOMA-IR), homeostasis model assessment-β (HOMA-β)] were compared before and after treatment.Results The effective rate in the observation was obviously higher than that of the control group(97.1% vs.75.0%, P<0.01).The hormone related indicators (LH, FSH, TEST, E2, SHBG) of the observation group significantly improved after treatment (P<0.01).The hormone related indexes (LH, TEST, E2) of the control group significantly improved after treatment (P<0.01), and the observation group was significantly better than the control group (P<0.01).The HOMA-IR and HOMA-β in the observation group were obviously lower than treatment before (P<0.01), and were significantly lower than those of the control group (P<0.01).Conclusion Metformin combined with Diane-35 can significantly improve the endocrine disorder and insulin resistance, and improve overall effective rate.
