补肺通络方对慢性阻塞性肺疾病肺肾气虚证患者免疫功能的影响

摘要

Objective To explore the effect of BufeiTongluo Fang on the immune function of chronic obstructive pulmonary disease (COPD)patients with lung and kidney deficiency syndrome. Methods From February 2015 to December 2016,80 cases of COPD patients were randomly selected and divided into the study group and the control group,40 patients in each group. 40 cases of healthy patients were selected ran-domly as the healthy control group. The control group was given routine treatment of western medicine. The study group patients were given western medicine and BufeiTongluo Fang. Both groups were treated for 14 days. The changes of pulmonary functional indexes and peripheral venous immu-nological indicators were compared before and after treatment. Results FEV1,FEV1/FVC,serum immunoglobulin,CD3 and CD4 T cell sub-group,CD4/CD8 ratios were significantly lower in COPD patients than that of healthy controls,CD8 T cells subgroup was significantly higher in COPD patients than healthy controls(P <0.01). After treatment,FEV1,FEV1/FVC,serum immunoglobulin,CD3 and CD4 T cell subgroup, CD4/CD8 ratios were significantly higher than those before treatment and control group;CD8 T cells subgroup was significantly lower than control group(P <0.05 or P <0.01). Conclusion COPD patients have pulmonary function and immune function abnormality,BufeiTongluo Fang has obviously improvement effect on lung function and immune function in COPD patients with lung and kidney deficiency syndrome. It is worthy of clinical application.%目的 探讨补肺通络方对慢性阻塞性肺疾病(COPD)肺肾气虚证患者免疫功能的影响.方法 前瞻性随机选择2015年2月至2016年12月收治的COPD患者80例,随机分为研究组、对照组,每组40例.选择同期健康人40例设为健康对照组.对照组给予常规西医治疗方案,进行吸氧、祛痰、解痉、平喘等常规治疗.研究组在对照组治疗基础上给予补肺通络方,两组治疗时间均为14 d.比较各组治疗前后肺功能指标、外周静脉血免疫学指标变化情况.结果 对照组及研究组1秒用力呼气容积(FEV1)、FEV1/用力肺活量(FVC)值、血清免疫球蛋白各指标水平、CD3及CD4 T细胞亚群数量、CD4/CD8值均明显低于健康对照组,CD8 T细胞亚群数量明显高于健康对照组(P <0.01).研究组治疗后FEV1、FEV1/FVC、免疫球蛋白各指标水平、CD3及CD4 T细胞亚群数量、CD4/CD8值明显高于治疗前、对照组治疗后;CD8 T细胞亚群数量明显低于本组治疗前、对照组治疗后;经比较差异有统计学意义(P <0.05或P <0.01).结论 COPD患者存在肺功能及免疫功能异常,补肺通络方对COPD肺肾气虚证患者肺功能有明显改善作用,亦能有效改善患者免疫功能,值得临床推广应用.
