输尿管镜钬激光碎石治疗嵌顿输尿管结石疗效观察

摘要

Objective:To evaluate the clinical therapeutic efficacy of Holmium laser lithotripsy and provide more theoretical reference for the clinical treatment of impacted ureteral calculus .Methods:130 patients with impacted ureteral calculus hospitalized from March 2012 to April 2014 were randomly divided into the control group and the observation group according to different surgical methods , with 65 cases in each group.The control group was treated with extracorporeal shock wave lithotripsy , while the observation group was treated with holmium laser lithotripsy .And then the complications , the clinical efficacy and recurrence rate were comparedbetween the two groups.Results:After treatment, the cured cases ,the effective cases and the ineffective cases of the control group were 13, 32 and 20 respectively, and the total effective rate was 69.23 %;while the cured cases, the effective cases and the ineffective cases of the observation group were 25 , 36 and 4 respectively, and the total effective rate was 93.85 %.There was a significant difference in the total efficiency between the observation group and the control group ( P <0.05); There were no serious complications in both groups but there were 6 cases of recurrence in the control group and the recurrence rate was 9.23 %.There was no recurrence in the observation group.The difference between the two groups was statistically significant ( P <0.05).Conclusion:Ureteros-copy holmium laser lithotripsy for the treatment of impacted ureteral calculus is significantly effective , which can not only greatly alleviate the suffer-ing of patients and improve the life quality of patients, but also has high security and low recurrence rate.Thus, It is worth promoting in the clinical treatment of impacted ureteral calculus.%目的：探讨输尿管镜钬激光碎石治疗嵌顿输尿管结石的临床疗效，为临床治疗嵌顿输尿管结石提供理论参考依据。方法：随机选取2012年03月至2014年04月期间就诊的嵌顿输尿管结石患者130例，据手术方式的不同随机分为对照组和观察组，每组65例，对照组采用体外冲击碎石治疗，观察组采用输尿管镜钬激光碎石治疗，比较两组患者的并发症、临床疗效及复发率情况。结果：经过治疗，对照组患者中显效13例，有效32例，无效20例，总有效率为69．23％；观察组患者中显效25例，有效36例，无效4例，总有效率为93．85％；观察组总有效率明显优于对照组，两组差异具有统计学意义（ P ＜0．05）；两组患者均没有严重并发症，对照组有6例复发，复发率为9．23％，观察组没有复发，两组差异显著具有统计学意义（ P ＜0．05）。结论：输尿管镜钬激光碎石治疗嵌顿输尿管结石具有显著效果，可以大大减轻患者的痛苦，提高患者生命质量，还具有较高的安全性以及极低的复发率，值得临床推广使用。
