目的 探讨应用局部切除术治疗早期低位直肠癌的适应证及疗效.方法 总结2001年1月至2005年12月间应用局部切除术治疗早期低位直肠癌的27例的临床资料,并结合近年来的国内外相关文献进行回顾性分析.结果 本组Tis期5例,T1期18例,T2期4例;全组无围手术期死亡病例,术后并发症发生率7.4%(2/27);平均随访时间6.8年,5年总体复发率14.8%(4/27),5年总体生存率88.9%(24/27).结论 应用局部切除手术治疗早期低位直肠癌,具有并发症发生率低和保留肛门的优点;在严格掌握适应证的前提下,可获得与根治性手术接近的远期疗效.%Objective To evaluate the efficacy of local resection on early stage of lower rectal carcinoma. Methods The clinical data from 27 patients with early stage of lower rectal carcinoma, admitted during the period from Jan. 2001 to Dec. 2005, were retrospectively analyzed. Results Among these patients, 5 were fallen into stage Tis, 18 into stage T1, and 4 into stage T2. No perioperative deaths occurred and the incidence of complication was 7. 4 ％ (2/27). The average period of follow-up was 6. 8 years,the 5-year recurrent rate was 14. 8 ％ and the overall 5-year survival rate was 88. 9 ％(24/27). Conclusion Lower complications and the preservation of anus are advantages of local resection, and the satisfied efficacy may be achieved as long as we consider indications carefully.
