神经内镜下椎管内肿瘤切除16例临床分析

摘要

目的 分析在神经内镜辅助下切除椎管内肿瘤的临床效果.方法 经M RI确诊椎管内硬脊膜下、髓外神经纤维瘤和脊膜瘤患者16例.肿瘤位于颈段2例、胸段3例、腰段11例.采用2-4 cm皮肤切口 ,鼻窥器显露椎管 ;在神经内镜辅助下切除肿瘤.结果 16例患者的椎管内肿瘤均全部切除.术后症状消失或明显改善 ,脊柱的稳定性无明显变化.结论 使用鼻窥器和神经内镜可减小脊柱的破坏 ,并可完全显露和切除椎管内肿瘤 ,手术安全、有效、微创.%Objective To analyze the outcomes of resection of spinal tumors assisted by neuroendoscope .Methods The intradural or extramedullary neurofibroma and meningioma were comfirmed by MRI in 16 patients .The tumors located at the cervical vertebrae in 2 cases ,the thoracic in 3 cases ,and the lumbar in 11 cases .The spinal canal was showed by nasal speculum via an incission of 2-4 cm .The neuroendoscope-assisted resection of the tumors was performed .Results The tumors were completely resected in 16 patients .Clinical symptoms were disappeared or improved after operation without impact on the stability of the spine .Conclusion Assisted by nasal speculum and neuroendoscope ,the resection of spinal tumors has the advantages of sufficient exposure ,complete resection ,safety and minimal invasion .
