慢性心力衰竭患者自我护理行为与心理弹性的调研

摘要

目的 研究慢性心力衰竭患者自我护理行为与心理弹性量表的关系,为临床护理提供数据参考.方法 选取2015年8月至2017年8月本院收治的慢性心力衰竭患者120例为观察对象.采用本院自制调查量表对患者的临床资料进行统计分析,主要包括体重、性别、学历、婚姻状况、吸烟、饮酒、运动以及心功能严重程度等.采用自我护理行为量表对患者自我护理行为情况进行分析,采用CD-RISC心理弹性量表对患者心理弹性情况进行调查.并作多因素Logistic回归分析.结果 男性、学历在高中及以上、经常运动的慢性心力衰竭患者心理弹性总分明显高于女性、学历在初中及以下、不经常运动的患者,心功能分级为Ⅰ～Ⅱ级、自我护理行为评分≥100分的慢性心力衰竭患者心理弹性总分均明显高于心功能分级Ⅲ～Ⅳ级、自我护理行为评分＜ 100分患者(均P＜ 0.05).经多因素Logistic回归分析可得:女性、学历在初中及以下、不经常运动以及心功能分级均为影响慢性心力衰竭患者心理弹性的独立危险因素,而自我护理行为评分是其保护性因素(均P＜ 0.05).结论 慢性心衰患者自我护理行为与心理弹性较差,且两者存在密切相关.%Objective To study the relationship between self-care behaviors and mental resilience scale in patients with chronic heart failure.Methods From August 2015 to August 2017,120 patients with chronic heart failure were selected as observation object.The clinical data of the patients were statistically analyzed by the self-made questionnaire of our hospital,including weight,sex,education background,marital status,smoking,alcohol consumption,exercise,and severity of cardiac function.The self-care behaviors of the patients were analyzed by the self-made self-care behavior scale,and the mental resilience of the patients was investigated by the CD-RISC mental resilience scale.Results The total score of mental resilience of male patients,patients with high school and above education background,patients who exercised frequently was significantly higher than that of female patients,patients with middle school and below education background,patients who did not exercise frequently;the total score of mental resilience of patients with cardiac function grade Ⅰ-Ⅱ,self-care behavior score ≥100 points was significantly higher than that of patients with cardiac function grade Ⅲ-Ⅳ,self-care behavior score ＜100 points (P＜0.05).The multivariate Logistic regression analysis showed that female,middle school and below education background,infrequent exercise,cardiac function grade were independent risk factors influencing mental resilience of patients with chronic heart failure,and self-care behavior was the protective factor (P＜0.05).Conclusion The self-care behavior and mental resilience of patients with chronic heart failure are poor,and they are closely related to each other.
