新式助吸乳贴促进乳头凹陷初产妇母乳喂养的临床应用

摘要

目的:探讨新式助吸乳贴促进乳头凹陷初产妇母乳喂养的临床运用价值.方法:将2016年1月至2017年4月在我院产科住院治疗的183例乳头凹陷初产妇随机分为治疗组和对照组,其中对照组82例初产妇采用常规的护理方案,观察组101例初产妇采用新式助吸乳贴护理方案,分别在产后3d,1个月及4个月后比较初产妇母乳喂养自我效能、纯母乳喂养率和产妇喂哺舒适度情况.结果:在自我效能感方面,对照组产后3d,1个月及4个月后的评分分别为40.55±4.52,48.49±5.21和50.49±4.67,观察组分别为40.31±4.36,79.58±13.09和90.60±14.05;在舒适度方面,对照组产后3d,1个月及4个月后的评分分别为7.96±1.09,7.22±0.96和6.22±0.94,观察组分别为8.08±1.07,6.16±0.94和2.91±0.49;产后3d时两组初产妇母乳喂养自我效能和舒适度均无明显差异(P＞0.05),产后1个月和4个月后观察组母乳喂养自我效能和舒适度明显优于对照组(P＜0.05);重复测量方差分析显示:两组产妇在自我效能和舒适度维度的时间因素、组间因素及交互因素差异均有统计学意义(P＜0.05);观察组产后1个月和4个月纯母乳喂养率显著高于对照组.结论:新式助吸乳贴可有效促进乳头凹陷初产妇母乳喂养成功率,提升产妇母乳喂养自我效能感,改善喂哺舒适度,值得临床进一步推广.%Objective:To explore the value of new type pressure pads in promoting breastfeeding in primipara of inverted nipple.Methods:A total of 183 cases of inverted nipple primiparas from January 2016 to April 2017 in our hospital were randomly divided into a treatment group and a control group.Among them,82 cases in the control group were treated with routine nursing program,101 cases in the treatment group were treated by the new negative pressure pads.Breastfeeding self-efficacy,breast-feeding rate and maternal feeding comfort conditions in the postpartum 3 days,1 month and 4 months were compared between the two groups.Results:The scores of self-efficacy in the control group after 3 days,1 month and 4 months were 40.55±4.52,48.49±5.21 and 50.49±4.67,while the observation group were 40.31±4.36,79.58±13.09 and 90.60±14.05.The comfort scores of the control group after 3 days,1 months and 4 months were 7.96±1.09,7.22±0.96 and 6.22±0.94,While the observation group were 8.08±1.07,6.16±0.94 and 2.91±0.49.The maternal breastfeeding self-efficacy and comfort of the two groups of were no significant difference at 3 days postpartum.The breastfeeding self-efficacy and comfort of the observation group were obviously better than that of the control group at 1 month and 4 months postpartum (P＜0.05).Repeated measures analysis of variance showed that the time,group and interaction factors of self-efficacy and comfort dimension of the two groups were significantly different (P＜0.05).The breast feeding rate of the observation group was significantly higher than that of the control group after 1 month and 4 months of postpartum.Conclusion:New type help pads can effectively promote the nipple breastfeeding success rate,can effectively improve maternal breastfeeding self-efficacy,improve feeding comfort,it is worthy of further promotion in clinical.
