颈部硬膜外腔注射皮质类固醇治疗神经根型颈椎病

摘要

背景 颈椎硬膜外腔皮质类固醇注射(epidural steroid injection,ESI)能有效地缓解神经根型颈椎病(cervical radiculopathy,CR)患者的疼痛. 目的 总结有关颈椎ESI在CR应用的文献资料. 内容 总结该方法的适应证、禁忌症,常用的皮质类固醇镇痛机制和常用剂型,两种常用的硬膜外腔入路,治疗效果,并发症以及并发症的预防. 趋向 临床医生应该采取积极有效的措施减少或者避免并发症的发生.更多的临床研究需探究治疗效果的预测因子以便提高疗效和减少治疗成本.%Background Cervical epidural steroid injection is performed effectively in pain relief to patients with cervical radiculopathy.Objective This article reviews the application of epidural steroid injection (ESI) on cervical radiculopathy.Content The indications and contraindications of ESI are described,in addition,the analgesic mechanisms and formulations of steroid,two major portals to cervical epidural space,efficacy,complication as well as precautions are introduced.Trend Clinicians should take more effort to reduce and avoid the complications caused by cervical epidural steroid injection.More researches are expected in identifying predictive factors to improve the outcomes and reduce the cost.
