目的:探讨社区肿瘤患者进行姑息治疗的临床效果.方法:选取肿瘤患者180例随机分为有可比性的两组,其中对照组88例,在院内接受抗肿瘤治疗后于院外正常生活;观察组92例,在对照组基础上于社区医疗机构为患者提供包括病情评估、护理、康复治疗以及临终关怀在内的全面姑息治疗.结果:观察组患者总体健康状况以及躯体功能、角色功能、情绪功能、认知功能、社会功能均明显优于对照组,疲劳、疼痛、失眠气促、食欲差以及便秘等症状评分明显低于对照组,两组比较差异明显,有统计学意义(P＜0.05);观察组患者经济状况与对照组相比差别不大,恶心呕吐以及腹泻等症状评分虽略高于对照组,但两组比较差异不明显,无统计学意义(P＞0.05).结论:社区姑息治疗对于肿瘤患者而言是一种有效而可行的治疗方式,值得临床推广.%Objective: To observe the clinical effect of palliative treatment in community cancer patients. Method: 180 tumor patients were divided into 2 comparable groups randomly, the control group ( 88 cases ) receiving normal life outside the hospital after anti-tumor therapy; the experiment group ( 92 cases ) providing comprehensive palliative care including condition assessment, care, rehabilitation therapy and hospice care on the basis of the control group for the patients. Result: Overall health status and physical function , role function, emotional function, cognitive function, and social function in the experiment group were better than the control group. And the symptoms score of fatigue, pain, insomnia, shortness of breath, poor appetite and constipation was lower than the control group. The difference between the 2 groups was obvious, which has statistical significance(P<0. 05 ). There was no great difference between the 2 groups in economic conditions. The symptom score of nausea , vomiting and diarrhea in the experiment group was slightly higher than that in the control group, but the difference between the 2 groups was not obvious, which has statistical significance(P<0. 05 ). Conclusion: Community palliative care is an effective treatment method for tumor patients, which is worthy to be promoted.
