肩三针配合手三阳经输穴贴敷治疗中风后肩手综合征临床观察

摘要

目的 观察肩三针配合手三阳经输穴贴敷治疗中风后肩手综合征的临床疗效.方法 将60例中风后肩手综合征患者随机分为2组.治疗组30例,予肩三针配合手三阳经输穴贴敷治疗;对照组30例,予常规针刺治疗.2组均治疗4周后评价疗效,并比较2组治疗前后日常生活能力、肩手部疼痛及上肢运动功能的变化,日常生活能力采用改良Barthel指数(BI)评定,肩手部疼痛采用视觉模拟评分法(VAS)评定,上肢运动功能采用Fugl-Meyer运动功能评价量表(FMA)评定.结果 治疗后治疗组总有效率93.3％,对照组总有效率73.3％,治疗组疗效优于对照组(P＜0.05);2组治疗后BI评分及FMA评分升高,VAS评分降低,与本组治疗前比较差异均有统计学意义(P＜0.05),且治疗组治疗后BI评分、VAS评分及FMA评分与对照组治疗后比较差异均有统计学意义(P＜0.05),治疗组患者日常生活活动能力、肩手部疼痛及上肢运动功能改善优于对照组.结论 肩三针配合手三阳经输穴贴敷治疗中风后肩手综合征临床疗效确切,可更好地提高患者日常生活能力,缓解肩手部疼痛症状,改善上肢运动功能,提高临床疗效.%Objective To observe the curative efficacy of shoulder three-needle combined with acupoint application of three yang meridians of hand on post-stroke shoulder-hand syndrome.Methods 60 patients with post-stroke shoulder-hand syndrome were randomly divided into two groups.30 cases in treatment group were treated by shoulder three-needle combined with acupoint application of three yang meridians of hand,and 30 cases in control group were treated by routine acupuncture treatment.The curative efficacy was evaluated after treatment 4 weeks in the two groups.The daily living ability,shoulder-hand pain and upper extremity motor function were compared before and after treatment in two groups.The daily living ability was evaluated by modified Barthel index (BI),shoulder-hand pain was evaluated by Visual analog scale (VAS),and the upper extremity motor function was evaluated by Fugl-Meyer motor function assessment (FMA).Results After treatment,the total effective rate in treatment group (93.3％) was superior to that in control group (73.3％,P ＜0.05).The scores of BI and FMA after treatment increased in two groups,while the score of VAS decreased.There were statistical differences on the scores of BI,VAS and FMA after treatment between the treatment group and control group after (P ＜ 0.05).The improvement of daily living ability,shoulder-hand pain and upper extremity motor function in the treatment group was better than that in the control group.Conclusion Shoulder three -needle combined with acupoint application of three yang meridians of hand has exact effects on post-stroke shoulder -hand syndrome,and it can preferably improve the daily living ability of patients,relieve shoulder pain symptom,improve upper extremity motor function and clinical efficacy.
