2型糖尿病合并无症状心肌缺血的危险因素分析

摘要

目的：探讨2型糖尿病合并无症状心肌缺血的危险因素。方法486例2型糖尿病患者，行冠状动脉CT血管成像检查，确诊合并无症状心肌缺血146例（ A组），未合并无症状心肌缺血组340例（ B组），分析两组患者的临床特征、实验室检查结果及冠状动脉CT血管成像检查的结果。结果 A组患者的平均年龄大于B组，糖尿病病程长于B组，高血压病史患者比例、男性比例、白蛋白尿患者比例及血肌酐水平高于B组（P＜0．05），两组患者的血总胆固醇、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇、甘油三酯、空腹血糖及糖化血红蛋白水平比较，差异无统计学意义（ P＞0．05）；logistic回归分析显示年龄、糖尿病病程、高血压病史、男性是2型糖尿病合并无症状心肌缺血的危险因素。结论年龄、糖尿病病程、高血压病史、男性是2型糖尿病合并无症状心肌缺血的危险因素。%Objective To explore the risk factors for type 2 diabetes mellitus complicated with asymptomatic myocardial ischemia .Methods Four hundred and eighty-six patients with type 2 diabetes mellitus underwent cardiac CT.They were divided into two groups ,146 cases complicated with asymptomatic myocardial ischemia in group A and 340 cases without asymptomatic myocardial ischemiain in group B .The analysis on the clinical characteristics as well as the results of laboratory examination and coronary CT angiography of two groups was conducted .Results The average age of patients in group A was older than that in group B ,the diabetes duration in group A was longer than that in group B,the proportions of hypertension history ,male cases and albuminuria cases ,and serum creatinine level in group A were higher than those in group B ( P<0.05 ) .There were no significant differences in the levels of total cholesterol(TC),low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C),triglyceride (TG),fasting plasma glucose(FPG)and glycosylated hemoglobin(HbA1c) between two groups(P>0.05).Logistic regression analysis suggested that age , diabetes duration , hypertension history and male were risk factors for type 2 diabetes mellitus complicated with asymptomatic myocardial ischemia .Conclusion Age,diabetes duration ,hypertension history and male are risk factors for type 2 diabetes mellitus complicated with asymptomatic myocardial ischemia .
