低温等离子射频消融术治疗早期喉癌及喉癌前病变的临床效果观察

         

摘要

目的 探讨低温等离子射频消融术治疗早期喉癌及喉癌前病变的临床效果.方法 将62例早期喉癌及癌前病变患者随机分为对照组30例和观察组32例,对照组患者接受传统喉裂开手术治疗,观察组患者接受低温等离子射频消融术治疗,比较两组手术时间、黏膜恢复时间、住院时间、疼痛评分、并发症发生率、复发率及嗓音声学功能恢复情况.结果 观察组术后黏膜恢复时间、住院时间、术后疼痛评分、并发症发生率短于或低于对照组(均P<0.05),两组手术时间相当(P>0.05);术后3个月,两组基频、基频微扰、振幅微扰等嗓音声学参数均低于术前(P<0.05),但两组间比较差异无统计学意义(均P>0.05);术后半年两组复发率比较,差异无统计学意义(P>0.05).结论 采用低温等离子射频消融术治疗早期喉癌及癌前病变,可有效改善患者嗓音声学功能,具有术后恢复快与疼痛感轻等优点.%Objective To investigate the clinical effect of low-temperature plasma radiofrequency ablation on early laryngeal cancer and precancerous laryngeal lesions .Methods A total of 62 patients with early laryngeal cancer or precancerous laryngeal lesions were randomly divided into control group(n=30) and observation group(n=32).Patients in the control group underwent conventional laryngofissure surgery,while patients in the observation group received low-temperature plasma radiofrequency ablation .The duration of operation ,time for mucosal recovery,hospital stay,pain score,incidence rate of complications ,recurrence rate,and recovery of voice acoustic function were compared between the two groups.Results The time for postoperative mucosal recovery,hospital stay,postoperative pain scores and incidence rate of complications in the observation group were shorter or lower than those in the control group (all P<0.05),both groups had similar duration of operation(P>0.05).At three months after surgery,the voice acoustic parameters,including fundamental frequency,fundamental frequency perturbation and perturbation amplitude ,decreased in the two groups compared to the preoperative (P<0.05),but there were no significant differences between the two groups (all P>0.05).After half a year of operation ,no statistically significant difference was found in the recurrence rate between the two groups ( P>0.05).Conclusion For patients with early laryngeal cancer or precancerous laryngeal lesions,low-temperature plasma radiofrequency ablation can improve the patients′voice acoustic function effectively,with advantages of rapid postoperative recovery and less pain .
