Objective To investigate the clinical manifestations and pathological features of gastric ulcer in elderly patients so as to provide reference for the diagnosis and treatment.Methods 170 patients with gastric ulcer were divided into 2 groups according to their age:observation group (80 elderly patients) and control group (90 middle-aged and young patients );the clinical manifestations and pathological features of the patients in the 2 groups were analyzed and compared.Results The main clinical manifestations of the patients in observation group were irregular abdominal pain and abdominal distension while those of the patients in control group were regular abdominal pain and acid reflux,there existed a difference of statistical significance in the occurrence between the 2 groups (P＜0.05);the gastric ulcers in observation group were mainly located in the stomach fundus and gastric body while those in control group were mainly located in gastric antrum,and the difference in the occurrence between the 2 groups was of statistical significance (P＜ 0.05);the occurrence of multi-site ulcer in observation group was higher than that in control group (P＜0.05);patients in observation group were more likely to be complicated with perforation,hemorrhage and pyloric obstruction,the occurrence of complications was higher than that in control group (P＜0.05).Conclusion The clinical manifestations and pathological features of gastric ulcers in elderly patients are different from those in young and middle-aged patients,which provides a basis for the diagnosis and treatment of gastric ulcers in the elderly.%目的 分析老年胃溃疡的临床表现和胃镜检查的病理特点,为该病的诊治提供参考.方法 选择胃溃疡患者170例,根据年龄分为观察组(80例老年患者)和对照组(90例中青年患者).分析并比较2组的临床表现和胃镜检查病理特点.结果 观察组临床表现特点是以不规律腹痛及腹胀为主,对照组则以规律腹痛及反酸嗳气为主,2组发生率差异有统计学意义(均P＜0.05);观察组胃溃疡病变部位主要在胃底及胃体部,对照组胃溃疡主要位于胃窦部,2组发生率差异有统计学意义(均P＜0.05),且观察组多部位溃疡的发生率显著高于对照组(P＜0.05);观察组易并发穿孔、出血及幽门梗阻,并发症发生率明显高于对照组(P＜0.05).结论 老年胃溃疡患者与中青年患者胃溃疡的临床表现特点和胃镜病理特征不同,可为老年胃溃疡的诊断及治疗提供依据.
