OBJECTIVE:To investigate the current status of pharmaceutical care in community , to probe into open problems and look for improvement measures . METHODS: Patients with chronic diseases , family doctors and pharmacists in community health service center in Changbai and Daqiao of Shanghai Yangpu District were chosen for questionnaire investigation , the contents of research included general situation of medication , desired pharmaceutical knowledge and provided pharmaceutical services .During May-Jul.2017, valid questionnaires of 384 from patients, 61 from family doctors and 21 from pharmacists were recycled .RESULTS:87.76％(337/384) of patients paid attention to community pharmaceutical service , with the favorite access to medicine knowledge of WeChat Public Account;93.44％(57/61) of family doctor demanded for pharmaceutical supporting from pharmacists , with the most desired for pharmaceutical knowledge of pharmacodynamics differences among the same kind of drugs .CONCLUSIONS:Pharmacists give full play of professional knowledge to take participate in community pharmaceutical service and family doctor team , which is essential for promoting rational drug application on community residents .%目的:了解社区药学服务的现状,探讨存在的问题,研究改进措施.方法:选择上海市杨浦区长白、大桥社区卫生服务中心的慢病患者、家庭医师和药师为研究对象,采取问卷的方式进行调查,调查内容包括用药情况、希望获得的药学知识与提供的药学服务.2017年5—7月,回收患者有效问卷384份,家庭医师61份,药师21份.结果:87.76％(337/384)的社区患者重视社区药学服务,其中以微信公众号宣传为患者最受欢迎的获取药物知识的途径;93.44％(57/61)的家庭医师也需要药师的药学支持,临床最希望获得同类药物药效学差异的相关药学知识.结论:药师利用自身药学知识参与到社区药学服务与家庭医师团队中,将为促进社区居民合理用药发挥重要作用.
