60岁及以上老年男性报告病例较高地区的艾滋病流行因素调查

摘要

Objective To find out the epidemiological risk factors in the area reporting high incidence of HIV/AIDS among old men aged 5= 60 years, and provide scientific evidence for targeted prevention and intervention. Methods In the areas reporting high case numbers or with high case proportion of HIV/AIDS of men aged ≥60 years, the cross-sectional survey was conducted on the high-risk behavior before infection with designed questionnaire. Results A total of 162 cases (from Duan in Guangxi, Nanchang in Jiangxi, Chengdu in Sichuan) were surveyed, 79.0% were transmitted by casual/commercial sexual partners, 11.1% were transmitted by homosexual partners, and the transmission rate through other infection routes was low. Before infection, 77.8% of the cases had commercial heterosexual contacts 30.9% had casual heterosexual contacts, 11.1% had homosexual contacts, the proportion of the cases with other high risk behaviors was low. The condom use rate was low at sex. Conclusion Unsafe sex was the major epidemiological risk factor in the area reporting high incidence of HIV/AIDS in old men aged ≥60 years. Attention should be paid on the sexual health and sexual safety among old men aged ≥ 60 years, and health education about HTV/AIDS shoulld be strengthened among old men.%目的 了解中国(未包括香港、澳门及台湾地区,下同)≥60岁老年男性报告病例较高地区的艾滋病流行因素,为制定有针对性的防治策略和干预措施提供参考依据.方法 在报告年龄≥60岁老年男性艾滋病病毒感染者及艾滋病患者报告数及构成比较高的地区,采用横断面调查方法,设计调查问卷,对存活病例感染前的高危行为进行调查.结果广西都安县、江西南昌市、四川成都市三个调查地点的162例调查对象中,79.0％的调查对象经临时/商业性伴传播,11.1％经同性传播,经其他途径传播的比例较低.77.8％的调查对象感染前有异性商业性行为,30.9％有临时性伴性行为,11.1％有同性性行为,有其他高危行为的比例较低.性行为时安全套使用频率低.结论 ≥60岁老年男性报告病例较高地区的主要流行因素是不安全性行为.≥60岁老年男性的性健康及性安全问题应引起全社会的关注,并采取干预措施,加强对老年男性的艾滋病宣传教育力度.
