目的 探讨心率因素在64排螺旋CT冠脉CTA前瞻性心电触发门控单扇区扫描与回顾性心电门控单扇区扫描模式间的对比.方法 回顾性筛选最高心率≤75次/分、心率波动≤10次/分的冠脉CTA病例229例(女106例,男123例),前门控115例,后门控114例,两组平均年龄分别为49.71±7.55(岁)、48.85±6.86(岁),平均心率分别为59.02±3.66(次/分)、60.18±5.51(次/分),平均心率差分别为2.18±1.92(次/分)、2.44±1.76(次/分),选择两组病例共有的9段冠脉进行图像质量主观5级评分.将两组病例间的年龄、心率、心率差、各段冠脉评分进行差异性t检验,P<0.05有统计学意义.结果 前后门控两组病例在年龄、平均心率与心率差上t检验P值分别为0.367、0.062、0.294,无显著性差异(P>0.05),对两组病例间各段冠脉图像质量评分进行对比分析有统计学价值,所选段冠脉间图像质量评分差异对比无显著性差异(P>0.05).结论 在最高心率≤75次/分、心率波动≤10次/分范围内,64排螺旋CT冠脉CTA前瞻性心电触发门控单扇区扫描模式与回顾性心电门控单扇区扫描模式获得的图像质量具有一致性的评估价值.%Objective To investigate the influence of heart rate (HR) on prospective and retrospective electrocardiogram (ECG)-gated 64-slice coronary CT angiography (CCTA). Methods Prospective (115 patients, mean age: 49.71 ±7.55 years, mean HR: 59.02 ±3.66 bpm, average HR fluctuation: 2.18 ±1.92 bpm) and retrospective (114, mean age: 48.85 ±6.86 years, mean HR:60.18±5.51 bpm, average HR fluctuation:2.44±1.76 bpm) ECG-gated CCTA of 229 patients (106 women, 123 men) with maximum HR≤75 bpm and HR fluctuation ≤10 bpm was retrospectively analyzed. 9 segments of coronary arteries were selected for image quality scoring. The patient age, HR, fluctuation of HR, and coronary artery image quality scores of both techniques were compared by t test. P<0.05 was defined as statistically significant. Results There was no significant difference in the patient age (t=-0.904, P=0.367), average HR (t=1.88, P=0.062), fluctuation of HR (t=1.052, P=0.294), or coronary artery image quality scores (P>0.05) between the two methods. Conclusion With the maximum HR≤75 bpm and HR fluctuation of HR≤10 bpm, prospective and ret-rospective ECG-gated CCTA has comparable image quality.
