目的 探讨细胞角蛋白( Cytokeratin,CK)14、17、19表达与银屑病病理改变的关系.方法 应用免疫组织化学方法检测银屑病30例(银屑病组)皮损处和健康对照者10例(健康对照组)皮肤组织CK14、CK17、CK19表达情况并进行分析比较.结果 健康对照组皮肤组织CK14主要在基底细胞层表达,而银屑病组皮损处CK14可扩展至基底上层,多为全层弥漫阳性表达；健康对照组皮肤组织CK17均阴性表达,银屑病组皮损处CK17均阳性表达且多在基底层以上；健康对照组CK19均在基底细胞层阳性表达,银屑病组皮损处CK19则多表达阴性.健康对照组和银屑病组CK14、CK17、CK19表达的平均光密度值分别为5.09±1.86和2.83±1.12、5.81 ±1.42和0、0.49±0.03和2.03±1.08,两组比较差异均有统计学意义(t=4.612,P=0.000;t =22.415,P=0.000;t =4.516,P =0.002).结论 CK14、CK17、CK19可能在银屑病发生与发展过程中发挥着重要作用.%Objective To explore the correlation of expression level of cytokeratin (CK) 14, 17 and 19 with pathological changes in psoriasis. Methods The expression level of CK 14, CK 17 and CK19 of the normal control group epidermis (10 cases) and psoriasis group lesion (30 cases) were detected by immunohistochemistry method, and comparative analysis was made. Results In group of normal controls, CK 14 was detected in basal layer, however, it was extended notably to suprabasal cell layers in the psoriasis group, most of which was diffusive expression of full-thickness. CK 17 had no expressions in the group of normal controls, but in psoriasis lesions, all examples were expressed, and many more in the basal layer. CK19 was detected in basal layer in the group of normal controls, but in the group of psoriasis, CK 19 was expessed more negatively. Normal control group and psoriasis group mean optical density value of CK14, CK17, CK19 were 5.09 ± 1. 86 and 2. 83 ± 1. 12, 5. 81 ± 1.42 and 0, 0.49 ± 0. 03 and 2. 03 ±1.08, respectively. There was significant difference between them (t =4.612, P =0.000; t =22.415, P =0.000; t =4.516, P =0.002). Conclusion CK14, CK17 and CK19 may play a crucial role in pathogenesis and development of psoriasis.
