高压氧联合阿托伐他汀对冠心病患者颈动脉斑块及血脂的影响

         

摘要

目的 观察高压氧(hyperbaric oxygen,HBO)联合阿托伐他汀对冠心病合并颈动脉粥样斑块、高脂血症患者的治疗效果,并与单独应用阿托伐他汀的疗效进行比较.方法 选择2011年2月-2012年2月我院收治的104例冠心病合并高脂血症、颈动脉硬化斑块,按随机数字表法分成两组.治疗组52例接受HBO联合阿托伐他汀治疗,对照组52例单独应用阿托伐他汀治疗,观察并对比两组治疗前后血脂指标及颈动脉斑块的变化情况.结果 治疗组应用HBO联合阿托伐他汀治疗1.5个月、3个月后,52例甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)及颈动脉内膜中层厚度(IMT)、斑块长径、斑块厚度较单纯应用阿托伐他汀的对照组有明显改善,差异均有统计学意义(P<0.05).治疗组治疗3个月后血脂治疗总有效率与对照组比较,差异亦有统计学意义(P<0.05).结论 HBO联合阿托伐他汀治疗可明显降低冠心病患者的血脂,并有效减轻动脉硬化,稳定易损斑块.%Objective To observe treatment effectiveness of HBO combined with atorvastatin on the patients with coronary heart disease combined with carotid arteries atheromatous plaque and hyperlipidemia, and to compare the effectiveness by atorvastatin alone. Methods A total of 104 patients with coronary heart disease combined with carotid arteries atheromatous plaque and hyperlipidemia during February 2011 and February 2012 in our hospital were divided into therapy group (n =52) and control group (n =52) by random digits table method. The therapy group was treated with HBO and atorvastatin, and control group was treated with only atorvastatin. Changes of blood fat indexes and carotid arteries plaque of the two groups before and after the treatment were observed and compared. Results The triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesferol (HDL-C), low density lipoprotein cholesterol (LDL-C) , carotid intima-media thickness (IMT) and long diameter and thickness of plaque of 52 patients in therapy group with HBO combined with atorvastatin for 45 days and 90 days obviously improved which compared with those in control group ( P < 0. 05). The difference of total effective rates in blood fat between therapy group and control group 3 months after treatment was of statistical significance ( P < 0. 05). Conclusion The treatment of HBO combined with atorvastatin can reduce the level of blood lipids and arteriosclerosis in patients with coronary heart disease and stabilize flimsy plaque.
