肾动脉烟囱技术在复杂瘤颈腹主动脉瘤腔内治疗的早期疗效

摘要

Objective To discuss the early therapeutic effect of renal artery chimney technique in endovascular aortic repair for abdominal aortic aneurysm with complex aneurysm neck. Methods The data of 9 patients with abdominal aortic aneurysm with complex aneurysm neck treated by renal artery chimney technique during July 2013 and October 2014 were retrospectively reviewed and analized. Results The rate of the surgical success was 100%. 9 patients were treated by stent grafting combined with chimney technique , which utilized 9 renal artery chimney grafts,including 4 balloon-expanding stents and 5 covered stents. Proximal type I endoleak occurred in 2 patients,and the endoleak disappeared in one after implanting a cuff, while the endoleak disappeared in one after kissing balooning. During the follow-up of 3-16 months (median 12 months), all patients survived without major neruologic complications, organ ischemia, stent migration, endoleak, and chimney graft occlusion. Conclusion Renal artery chimney technique is safe and effective in endovascular repair for abdominal aortic aneurysm with complex aneurysm neck during short-term follow-up , whereas the long-term outcome and complications need to be further studied.%目的:探讨肾动脉“烟囱”技术在复杂瘤颈腹主动脉瘤腔内修复术术中的早期疗效。方法回顾性分析2013年7月至2014年10月9例肾动脉“烟囱”技术治疗复杂瘤颈的腹主动脉瘤患者资料。结果手术成功率100%,9例患者通过“烟囱”加主体支架处理,共植入肾动脉“烟囱”支架9枚,其中4枚球扩支架,5枚覆膜支架。其中2例发生近端Ⅰ型内漏,1例辅助Cuff 后内漏消失,1例选用顺应性球囊对吻扩张后内漏消失。随访3~16个月(中位时间12个月),无患者死亡,未见严重神经系统并发症及脏器缺血发生,未见支架移位、明显内漏或“烟囱”支架闭塞。结论肾动脉“烟囱”技术在复杂瘤颈腹主动脉瘤腔内治疗的早期疗效确切,但其远期疗效及并发症需要进一步研究来证实。
