彩色多普勒超声结合三维超声诊断胎儿宫内窘迫的临床价值分析

摘要

目的 探讨彩色多普勒结合三维超声对诊断胎儿宫内窘迫的临床意义及特异性,提早发现胎儿窘迫的原因和情况.方法 通过彩色多普勒检测胎心、脐动脉和大脑中动脉血流参数,结合三维重建胎儿脐带血管,观察脐带形态、绕颈周数及空间转向,利用实时三维观察胎动形态及颜面部表情,更准确地做出诊断.结果 胎心率异常、脐动脉和大脑中动脉血流参数异常是胎儿缺氧和窘迫的表现,脐带缠绕、羊水过少和过期妊娠是造成胎儿窘迫的主要原因,三维重建更利于寻找窘迫的原因,超声诊断与产后符合率为100％.结论 彩色多普勒结合三维超声可以及时准确地诊断胎儿宫内窘迫的原因和程度,为临床诊断观察和选择合适的分娩方式提供参考,对优生优育具有重大的临床意义.%Objective To investigater clinical significance and specificity of color Doppler combined 3D ultrasound in the diagnosis of fetal distress in uterus. Methods To detect fetal heart, umbilical artery and middle cerebral artery parameters,Combined with three-dimensional reconstruction of fetal umbilical cord vessel, morphology, number and spatial turn of umbilical cord around neck,using real-time 3D to observe fetal movement and facial expressions more accurately. Results Abnormal fetal heart rate, fetal umbilical artery and middle cerebral artery blood flow parameter, Facial abnormalities are performance of fetal hypoxia and distress,Cord entanglement, oligohydramnios and prolonged pregnancy are the main causes of intrauterine fetal distress, The coincidence rate ultrasonographic diagnosis and postnatal diagnosis is 100%. Conclusions Color Doppler combined 3D ultrasound can accuratly diagnose the causes and degree of intrauterine fetal distress, and provide reference to select an appropriate mode of delivery.
