肺动脉/主动脉血流时间间期比例系数对定量肺动脉压的价值

摘要

The time interval indexes of acceleteration time/ejection time(AT/ET),preejection pulmonary and aortic blood flow in 17 Cases with normal and 33 with elevated pulmonary arterial Pres-sures were recorded by pulsed Doppler echocardiography to calculate ratios of pulmonary to aortic flow time intervals(RAT/ET,RPEP/AT,PA F/AOF,and Race).The estimated pulmonary artenal pressures(PAPE)were defined as the products of these ratios and the brachial arterial pressures measuredby a cuff sphygmomanolneter.The results showed that PAPE correlated sighificantly with PAP measuredby cardiac catheterization(r=0.65 0.76,P<0.001).In patients with pulmohary hypertension,there was aclose correlation between PAPE estimated by PAF/ AOF and PAP measured by cardiac catheterization(r-0.91,P<0.001).Thus、the product of PAF/AOF and the brachial arterial Pressures provides a new index for quantitative assessment of pulmonary arterial pressures%本文在17例肺动脉正常(A组)和33例肺动脉高压(B组)的心脏病患者中,应用脉冲波多普勒超声技术测量了肺动脉和主动脉血流的下列时间间期:加速时间比值(AT/ET)、心室射血前时间加速时间比值(PEP/AT)、血流平均加速(acc)和F值(F=ace×PEP/ET),进而计算肺动脉/主动脉血流时间间期比例系数(RAl/ET、RPET/AT、RACC、PAF/AOF),并以各项比例系数与肱动脉压的乘积作为肺动脉压的估测值.结果显示:RAT/ET、RpET/AT、PAF/AOF、Racc估测的肺动脉压与心导管相关良好(r=0.65 0.75),在B组患者中,PAF/AOF估测的肺动脉压与心导管测值相关最佳(r=0.91),表叫肺动脉/主动脉血流时间间期比例系数为无创性定量肺动脉压提供一种有价值的新方法.
