背景：一期人工关节置换治疗活动期髋、膝结核疗效显著，但人工假体的存在是否会增加结核复发的风险仍是争论的焦点。目的：综述活动期髋、膝结核行一期人工关节置换的基础与临床研究进展。方法：由第一作者检索CNKI、维普数据库、万方数据库和Pubmed数据库相关文献。中文检索词为“髋结核，膝结核，结核杆菌，假体，关节成形术，置换”；英文检索词为“hip tuberculosis，knee tuberculosis， mycobacterium tuberculosis，prosthesis，arthroplasty ，replacement”。检索时间范围：1985年1月至2012年10月。初步检索到文献258篇，根据纳入和排除标准，最终选择55篇文献进行综述。结果与结论：一期人工关节置换是治疗活动期髋、膝结核的有效方法，假体的存在并不影响抗结核药物对结核杆菌的杀灭，同时大量分子生物学方面的研究也为临床提供了坚实的理论基础。随着研究的深入和医师经验的积累，一期人工关节置换必将越来越为患者和医师所共同接受。% BACKGROUND: Efficacy of one-stage arthroplasty for patients with active hip and knee tuberculosis is significant, but whether the presence of prosthesis wil increase the uberculosis recurrence risk is stil the focus of debate. OBJECTIVE: To review the basic and clinical research progress of one-stage arthroplasty for patients with active hip and knee tuberculosis. METHODS: A computer-based online search of CNKI database, VIP database, Wanfang database and PubMed database between January 1985 and October 2012 was performed by the first author to search related articles with the key words of “hip tuberculosis, knee tuberculosis, mycobacterium tuberculosis, prosthesis, arthroplasty, replacement” in English or Chinese. A total of 258 literatures were primarily selected. According to the inclusion and exclusion criteria, final y 55 articles were included for review. RESULTS AND CONCLUSION: One-stage arthroplasty is an effective method for the patients with active hip and knee tuberculosis. The presence of prosthesis wil not affect the anti-tuberculosis drugs to kil mycobacterium tuberculosis; meanwhile, the large amount of molecular biology researches also provides a solid theoretical foundation for clinical treatment. With the deepening of research and surgeon experience accumulating, one-stage arthroplasty for the patients with active hip and knee tuberculosis wil increasingly common.
