蛛网膜下腔出血后患者脑血管痉挛与血浆内皮素血管紧张素及一氧化氮变化的相关性分析

         

摘要

Objective to study the relationship between cerebral vasospasm and plasma endothelin,angiotensin,nitric ox-ide in patients with subarachnoid hemorrhage.Methods 25 patients with subarachnoid hemorrhage were selected as experiment group,and 7 healthy patients with the same age as control group.The plasma endothelin,angiotensin and nitric oxide were de-tected at less than 24 hours,the fourth day,seventh day and fourteenth day,and relationship between them and the cerebral vasospasm was analyzed.Results The levels of plasma endothelin,angiotensin and nitric oxide in experiment group were in-creasing gradually in 24 hours,continued at the highest level for 4~5 days,the levels of plasma endothelin and angiotensin were decreasing at the seventh day,however,the levels of the above two were still significantly more than control group(P<0.05),the level of nitric oxide decreased to normal after seven days in experiment group;the levels of plasma endothelin in ex-periment group at the four time points were all significantly more than those of the control group(P<0.05),but the level of ni-tric oxide were significantly more only at the fourth and seventh day(P<0.05).Conclusion Plasma endothelin,angiotensin and nitric oxide may play an important role in the course of cerebral vasospasm.%目的:探讨患者在蛛网膜下腔出血后血浆内皮素、血管紧张素以及一氧化氮的变化及与脑血管痉挛的关系。方法随机选择蛛网膜下腔出血患者25例为实验组,与同时期同年龄段的健康人7例作为对照组,分别测量发病<24 h、第4天、第7天和第14天的血浆内皮素、血管紧张素以及一氧化氮水平的变化,并比较血浆内皮素与一氧化氮在两组患者4个时间点的差异,分析与脑血管痉挛发生率的关系。结果实验组患者体内的血浆内皮素、血管紧张素以及一氧化氮水平在24 h内慢慢升高,并持续在最高水平达4~5d;7d 后血浆内皮素、血管紧张素开始下降,但2周后依旧保持比对照组高(P<0.05),实验组患者7 d后一氧化氮降至正常水平;脑血管痉挛组患者的血浆内皮素在各时间点均高于对照组(P<0.05),一氧化氮仅在第4 d末和7 d末高于对照组(P<0.05)。结论在脑血管痉挛发病过程中,血浆内皮素、血管紧张素及一氧化氮起着至关重要的作用。
